Essential Duties and Responsibilities
Client Assessment and Resource Navigation
- Conduct intake interviews and assess clients' health-related social needs.
- Connect clients to community resources and public benefits, including:
- Housing and homelessness prevention services
- Primary care and health insurance enrollment
- Mental health and behavioral health services
- Food assistance and nutrition programs
- Employment and workforce development
- English language (ESL) and educational programs
- Transportation assistance
- Childcare and family support services
- Financial assistance and other social services
- Assist clients in completing applications and accessing available resources.
- Follow up with clients to ensure successful connection to services and address any barriers.
Care Coordination
- Coordinate referrals with healthcare providers, community organizations, and government agencies.
- Advocate for clients to ensure equitable access to services.
- Build and maintain collaborative relationships with community partners.
Documentation and Reporting
- Maintain accurate and timely documentation of client interactions, referrals, and outcomes in Apricot or other case management systems.
- Track referrals and resource utilization.
- Assist with data collection, reporting, and program evaluation.
Community Outreach
- Participate in community outreach, health fairs, workshops, and educational events.
- Promote CIN services and educate community members about available resources.
- Develop and maintain an up-to-date directory of community resources.
Qualifications
- Associate's or Bachelor's degree in Human Services, Social Work, Public Health, or a related field preferred.
- Minimum of one year of experience in resource navigation, case management, community outreach, or social services.
- Experience working with refugee, immigrant, asylum-seeking, or underserved populations preferred.
- Knowledge of community resources, healthcare systems, and public benefits.
- Strong communication, organizational, and problem-solving skills.
- Experience using case management databases such as Apricot is preferred.
- Ability to maintain confidentiality and work with individuals from diverse cultural backgrounds.
- Multilingual skills (French, Swahili, Lingala, Spanish, Arabic, or other community languages) are strongly preferred.

Physical Requirements
- Ability to travel within King County for client appointments, outreach activities, and community events.
- Ability to sit, stand, and use a computer for extended periods.
- Valid driver's license and reliable transportation are preferred.
