Rehab Intake Coordinator/Admission Liaison - REHAB - INPATIENT

DEFINITION:The case assessment specialist is responsible for evaluating and coordinating potential patients through the continuum of care into rehab and proactively plans and functions as the liaison between patient/family and/or referral sources with the rehab hospital; build and maintain markets for rehab programs, services, and physicians and to attract and satisfy patients.

MINIMUM QUALIFICATIONS:

Licensed as a Registered Nurse in the State of Oklahoma.

Valid Oklahoma drivers license and automobile insurance verification

PREFERRED QUALIFICATIONS:

Must be able to communicate effectively in English (Verbal/Written)

2 years experience in a health care setting;rehabilitation facility preferred

Previous experience in an inpatient rehabilitation facility.
