JobSummary:Â
TheCareCoordinator isresponsible for supporting ourmembersastheynavigatethe complexhealthcare delivery system,ensuring every member receives theright care in theright setting. Thisindividualwillserveasa guideforthemember andtheirfamiliesto ensurethe member meets withthe rightspecialists,insideoroutsidethe center,inatimelymanner.Anidealcandidatewill haveadeepunderstanding of the healthcare ecosystem,havestrongattentionto detail,andbe drivenbyapassion for providingqualitycareto seniors.Â

Duties/Responsibilities:Â
- PartnerwithArchWellHealth'smulti-disciplinarycareteamtogainadeepunderstandingof eachmember's careneeds,includingidentifying gapsand barriersto treatmentÂ
- Coordinatereferralstoexternalproviders, vendors, and servicestoensurecare needsaremetÂ
- Completeproperdocumentationformemberreferralsto ensurerecordsareaccurateandcurrentÂ
- Build and nurture relationships with members andtheir families to successfully facilitate care coordinationÂ
- Carry outall activities andprovide ongoingmonitoringas outlinedin amember'scare plan toensure routine followup and progressionis made on care plan goalsÂ
- AdheretoHIPAAstandardsandconfidentialityofprotectedhealthinformation(PHI)Â
- Promoteclearcommunicationamongcareteamandtreatingclinicianstoensurepositive member outcomes Â
- Assist members in problem solving potential issues related to the health care system, financial or social barriers (e.g., request interpreters as appropriate, transportation services or prescription assistance). Â
- Be the system navigator and point of contact for members and families, with patients and families having direct access for asking questions and raising concerns. Â

- Deepunderstandingofthehealthcareecosystem, ideally with experience in benefits and insurance authorizationÂ
- Passionforhelping seniormemberssuccessfullycarryoutcareplansÂ
- Ability towork effectively with diverse populationsincluding staff, providers, members,family members, insurance carriers, vendors, and the publicÂ
- Basic medical terminologyÂ
- Understanding of Medicare criteria for equipment and suppliesÂ
- Knowledge of CPT and ICD-10 codesÂ
- Workingknowledgeof specialistsandcommunityresourcestoassistthememberÂ
- Strongcommunication and rapport-building ability amongan interdisciplinary teamÂ
- Ability to multi-task and manage competing prioritieswith high attention to detailÂ
- Proficient PC skills to include Microsoft OfficeÂ
- Fluency in Spanish, or languages spoken by the people inthecommunitieswe serve (where necessary)Â
Education and Experience:Â
- H.S. Diploma or equivalent required; degree orcollege levelcoursework inhealthcarepreferredÂ
- 2 years of experience inreferral management or care coordinationÂ
- Medical Assistant ormedicaladministrative experience is helpfulÂ
- Experience with EMR systems, ideally eClinicalWorks or similarÂ
