Hope Is Alive Ministries is a faith-based nonprofit recovery organization dedicated to transforming lives through Jesus Christ. Founded on the belief that lasting recovery is possible, HIA operates across six states (Oklahoma, Kansas, Missouri, Colorado, Texas, and North Carolina), serving individuals and families affected by addiction through residential recovery programs, community outreach, and discipleship.

Position Summary
The Family and Aftercare Coordinator serves as the primary point of connection between The Healing Center and the approved family members and support persons of its residents. Throughout each resident's 45 day stay, this role coordinates family involvement, helps families understand the Healing Center experience, connects them with Hope Is Alive resources, and maintains consistent communication within the limits of resident consent and privacy. The coordinator also works directly with each resident to develop an aftercare plan ahead of their transition from The Healing Center, coordinating housing, community support, and next steps, and leads alumni follow-up and engagement after discharge. This is a non-clinical role focused on relationships, communication, coordination, documentation, and continuity of support. It does not provide therapy, clinical assessment, or treatment planning.

Key Responsibilities
The following are the primary duties and responsibilities of this position. This list is not exhaustive and may be adjusted to support organizational needs.
Family Engagement and Communication
- Serve as the primary Healing Center contact for approved family members and support persons
- Confirm that a signed, current resident release of information is on file before any contact with a family member or support person, and limit what is shared to the scope of that consent
- Make an introductory connection with each resident's approved family contact within the resident's first week at The Healing Center
- Explain the purpose, structure, expectations, and general experience of the 45 day program
- Maintain a consistent rhythm of communication with families throughout the resident's stay, sharing appropriate updates while honoring resident privacy and Healing Center information-sharing expectations
- Help families understand how to support their loved one without attempting to control, rescue, or take responsibility for the resident's recovery
- Receive questions and concerns from families, direct them to the appropriate staff member, and promptly communicate concerns requiring leadership involvement to the Director of Operations
- Refer clinical questions, safety concerns, and crisis disclosures from residents or families to the clinical team or Healing Center leadership immediately, following established protocol
Hope Is Alive Resources and Family Events
- Educate families about relevant Hope Is Alive programs, ministries, and resources, and connect them with Finding Hope and other appropriate family support resources
- Explain the Hope Is Alive housing and community opportunities that may be available to residents after completing The Healing Center
- Help families understand the importance of continued structure, accountability, spiritual growth, and healthy community after discharge
- Maintain working knowledge of current Hope Is Alive programs so families receive accurate and consistent information
- Communicate Family Weekend dates, expectations, registration information, and participation requirements to approved family members, and coordinate family attendance
- Assist Healing Center leadership and staff with the facilitation of Family Weekend, and support family participation in graduations, send-offs, and other approved events
Aftercare Planning and Transition
- Meet with each resident early in their stay to begin developing an individualized aftercare plan, and revisit it regularly so the plan is complete and confirmed before discharge
- Develop each plan in collaboration with the resident's assigned Healing Strategist and Healing Center leadership, building on the resident's Relapse Prevention Plan and the clinical team's recommendations
- Ensure each aftercare plan addresses housing, transportation, recovery community and church connection, continuing care referrals and appointments, family expectations, and immediate next steps
- Coordinate referrals and warm handoffs to Hope Is Alive housing staff, continuing care providers, and other approved resources
- Ensure the resident and, with the resident's consent, approved family members understand the aftercare plan and their part in it
- Track outstanding transition needs and follow up with the individuals responsible for completing them
- Document the confirmed aftercare plan in the resident record and communicate unresolved concerns to the Director of Operations
- Support both planned and unplanned discharges according to established Healing Center policies and leadership direction
Alumni Coordination
- Serve as the primary coordinator for Healing Center alumni communication and engagement
- Complete post-discharge follow-up according to the established alumni contact schedule, encouraging alumni to remain connected to healthy community, accountability, spiritual growth, and recovery support
- Oversee approved alumni communication channels, including the Healing Center alumni GroupMe
- Coordinate alumni gatherings, service opportunities, and speaking opportunities, and invite and help prepare alumni to participate in appropriate Healing Center events
- Maintain current alumni contact information and document follow-up efforts
- Notify Healing Center leadership when an alumnus reports a significant concern, requests additional support, or appears to need immediate assistance
Documentation and Team Communication
- Maintain accurate and timely records of family contacts, resource referrals, aftercare planning, alumni follow-up, and significant concerns
- Protect resident, family, and alumni confidentiality in accordance with applicable privacy laws and Healing Center policy
- Provide regular updates to the Director of Operations regarding family engagement, upcoming transitions, alumni activity, and unresolved needs
- Participate in relevant staff meetings and resident transition discussions, and communicate professionally and consistently across Healing Center and Hope Is Alive departments
- Track and report outcomes and engagement information as requested by leadership, such as family contact rates, aftercare plan completion, and post-discharge follow-up
- Perform other duties as assigned
Qualifications
Required
- High school diploma or equivalent
- Strong verbal and written communication skills, including comfort initiating phone calls and handling difficult or emotional conversations with professionalism, patience, and good judgment
- Able to protect confidential information and follow established boundaries regarding what may be shared with family members
- Organized and dependable, able to manage multiple residents, families, aftercare plans, and follow-up schedules at the same time
- Able to document communication, referrals, and transition arrangements accurately and promptly, with working proficiency in standard office software
- Valid driver's license
- Willing to work occasional evenings and weekends for Family Weekend, alumni events, graduations, and other scheduled activities
Preferred
- Experience in discharge planning, aftercare coordination, or alumni programs, including maintaining long-term relationships with former program participants
- Two or more years of experience in case management, care coordination, family support, admissions, resident services, customer relations, or a similar role requiring consistent outreach and follow-up
- Experience working within a Christian ministry, church, nonprofit organization, recovery program, or residential environment
- Experience communicating with families during challenging or emotionally difficult circumstances
- PRSS certification or related college education
- Familiarity with Hope Is Alive, Finding Hope, or recovery support communities, and an understanding of healthy family boundaries in recovery
- Comfortable speaking to groups, facilitating family discussions, and representing The Healing Center at events

Work Environment
- On site at The Healing Center residential facility
- Full-time; standard schedule is 8:30 AM to 5:00 PM, Monday through Friday, with occasional evenings and weekends for Family Weekend, alumni events, and graduations
- Regular exposure to emotionally difficult conversations with residents, families, and alumni
- Occasional local travel for alumni gatherings and events
Physical Demands
- Ability to sit and work at a computer and on the phone for extended periods
- Ability to move throughout the facility and assist with event setup, including occasionally lifting up to 25 pounds
- Ability to communicate clearly by phone and in person
