Marie Curie is the UK's leading end of life charity. We are the largest non-NHS provider of end-of-life care in the UK and the only provider operating across all four nations. Through our nursing, hospice and support services, research and campaigning, we work to ensure everyone has the best possible quality of life at the end of life.

End of Life Care Clinical Nurse Specialist (PCN Embedded)

Help people have the conversations that matter most.

Every day, people living with an illness they are likely to die from face uncertainty about the future. Too often, support comes only when a crisis happens.

This role is about changing that.

We are looking for a compassionate and motivated nurse to join our team as an End-of-Life Care Clinical Nurse Specialist, embedded within NEXUS Primary Care Network (PCN), Exeter.

This is an exciting opportunity to be part of an innovative service that helps people receive the right support earlier, giving them more time to plan, make informed choices and receive care that reflects what matters most to them.

You will build trusted relationships with people, families and professionals, supporting meaningful conversations about future care, coordinating support and helping ensure wishes and preferences are understood and respected.

A key focus of the role will be supporting the timely completion of Treatment Escalation Plans (TEPs), ensuring individuals are empowered to make informed decisions about their care, treatment and future wishes. You will champion advance care planning and work collaboratively with patients, families and professionals to ensure care is aligned with what matters most to each person.

Working alongside GP practices, community services and Marie Curie colleagues, you will help improve experiences of care, reduce avoidable crises and support people to remain in their preferred place of care wherever possible.

Most importantly, you do not need to see yourself as an end-of-life care specialist already. If you are an experienced nurse who is passionate about person-centred care, curious to learn and committed to making a difference, we will support you to develop the knowledge, confidence and skills needed to thrive in the role.

Hours: 37.5 hours per week, Monday to Friday

Contract: Permanent (flexible working to be discussed)

Salary: Band 6, ÂŁ39,959 - ÂŁ48,117 per annum (aligned to NHS Agenda for Change)

Location: Exeter Primary Care Network and community settings

We are looking for someone who:

  • Is passionate about improving the experience of people approaching the end of life and those important to them.
  • Believes every person deserves care that reflects their wishes, values and priorities.
  • Can build trusting relationships with patients, families and professionals.
  • Is confident having sensitive conversations, or willing to develop these skills.
  • Enjoys working collaboratively across teams and organisational boundaries.
  • Is motivated to develop their knowledge and expertise in end-of-life care.
  • Can work autonomously whilst remaining connected to a supportive wider team.
  • Shares our values and commitment to compassionate, equitable and person-centred care.

In return, we offer:

A supportive learning environment with access to specialist mentorship, professional development opportunities and the chance to help shape an innovative approach to end-of-life care within primary care and local communities.

This is more than a nursing role. It is an opportunity to influence care across whole populations while making a meaningful difference to individuals and families every day.

We also honour your NHS pension and NHS annual leave entitlement, making your transition as smooth as possible! (T&C’s apply)

If you believe every person deserves the opportunity to talk about what matters most, make informed choices and receive care that reflects their wishes, we'd love to hear from you.
