About the Role: 

The Doula Surgery Integrated Health Navigator will provide assistance to transgender and gender non-binary (TGNB) patients in navigating medical and supportive service systems related to recovery care.

General responsibilities include:

• Perform assessments and develop a personalized care plan.
• Develop relationships with community organizations supporting TGNB communities.
• Conduct outreach activities to include community visits and consults.
• Follow up appointments.
• Assist in the development of health education materials focused on the health needs and concerns of TGNB communities.

Work Schedule/Benefits:

Callen-Lorde is open Monday - Saturday. This is a Monday-Friday work schedule, and some evening/weekends hours may be required.

Qualifications

• High school diploma or GED preferred.
• Direct TGNB service experience or relevant TGNB life experience.
• Experience in social services, case management and/or entitlements work preferred; specifically with TGNB, LGB, and HIV-affected communities.
• In-depth knowledge of health and social issues facing the TGNB community and familiarity with service and resources available.
• Knowledge of transgender surgical procedures, WPATH standards of care and NY state TGNB health insurance requirements preferred.
• Experience in social services, case management and/or entitlements work preferred.
• Strong computer competency, including knowledge of MS Office programs, internet-based research, and capacity to work with Electronic Medical Records and Computerized Appointment

• Bilingual (Spanish/English) preferred.
