Position Summary
The Director of Enhanced Care Management is the operational director of Leadership, innovation and Accountability (LIA) for a subset of care streams and all Center in a dyadic leadership structure with the clinical/medical director of WellSpace Health Services. The Director of Enhanced Care Management is responsible for promoting organizational alignment, leadership development, operational accountability, access, proceptivity, and exceptional patient/client care across multiple care streams and all Centers. They will work to integrate the operations of their care streams across all Centers, department and programs, within the WellSpace Health organizational structure. This position functions as a strategic integrator, guiding Center Managers (CM’s) other LIA’s and Vice Presidents to ensure coordination, communication, collaboration and compliance for their care streams to fulfill WSH organizational initiatives to maintain access to exceptional patient/client care and financial stability.
The Director of Enhanced Care Management (ECM) provides strategic leadership and oversight of the ECM team, with a focus on performance, integration, accountability, innovation, and the delivery of high-quality, equitable care. The Director ensures alignment with clinical standards, regulatory requirements, and organizational goals while promoting operational excellence and continuous improvement. The role fosters strong collaboration across care teams, departments, and community partners to enhance coordination of care and improve outcomes for Enhanced Care Management members.
Required Qualifications
- Bachelor’s degree in Health Administration, Business, Public Health, Social Work or relegated field required.
- Minimum 5-7 years of progressive leadership experience within an FQHC, CCBHC, community health, mental health, case management, or nonprofit setting.
- Demonstrated success managing teams, projects, and interdepartmental initiatives.
- Strong leadership, communication, and team development skills.
- Demonstrated ability to manage grants funding, monitor budgets, and prepare reporting for government and funding agencies and compliance across multidisciplinary teams.
- Oversee and participates in the interviewing and hiring process, ensuring compliance with all agency and regulatory policies, practices, and laws.
- Demonstrated ability to oversee and participate in multidisciplinary team meetings, coordinating client and family evaluations, program admissions, and discharge planning.
- Strong communication, scheduling and compliance understanding; ability to multitask in mid-size to large size teams.
- Demonstrated knowledge and skill in providing evidence-based support services.
- Commitment to a community health philosophy and demonstrate adherence to WSH Patient Centered Health Home (PCHH) team-based model of care.
- Proficiency in EHR systems, data analytics, and performance dashboards or ability to become so within 6 months of employment.
- Ability to communicate effectively and work with individuals from a wide variety of cultural, ethnic, and socio-economic backgrounds, and possess sensitivity to their health problems.
- May be required to travel within the Sacramento and Placer County Region 10%
- Established understanding of FQHC regulations, CCBHC regulations, HRSA standards, SAMHSA standards, HIPAA, OSHA, HEDIS/UDS metrics, The Joint Commissions, FTCA risk management, and California Title 22 requirements and continuous readiness systems or ability to become so within one year of employment.
- Commitment to HIPAA, and ability to abide by standards of professional ethics and maintain confidentiality and Just Culture principles.
- Current BLS and first aid certificate or willingness to be trained (required within the first (90) ninety days of employment).
- Support the standards of High Reliability.
- Complete WellSpace Health Leadership trainings such as DELTA, LEAD, or similar within three years of hire.
Essential Responsibilities
- Works in Dyadic partnership with the Clinical/Medical Director/Center Manager to ensure high-quality care delivery for subset of WellSpace Health care streams.
- Serve as the Organization subject matter expert for their care streams operations, providing strategic oversight and operational guidance to ensure programmatic excellence.
- Act as an innovator of a subset of WellSpace health care streams by focusing on maintain access, growing lines of services, and expanding population access to services tough integration and growth.
- Act as an accountability leader for a subset of WellSpace Health Care streams by having staff take responsibility for their actions, serving reasonable objectives and deadlines and creating a culture of ownership over outcomes.
- Assisting with guidance and orientation/training of new providers and support staff in areas of charting, computer & EHR training, and center workflows.
- Partner with Center Manager to plan and lead center meetings and engage in collaborative meetings with external partners.
- Works with Center Manager to maintain productivity and other quality of care metrics for Enhanced Care Management at appropriate levels, in accordance with WSH initiatives, policies, and budgetary guidelines.
- Monitors program-level data and performance metrics to ensure compliance with Enhanced Care Management contractual requirements and organizational quality standards, identifying trends and supporting corrective action planning as needed.
- Ensure compliance of the Individualized Care Plan (ICP).
- Participates in program evaluation and continuous quality improvement activities, including data collection, analysis, and implementation of process improvements to enhance program outcomes.
- Collaborates with internal departments and cross-functional teams (including Training, Fiscal, Health Initiative programs, and technical assistance partners) to support coordinated implementation of program requirements, reporting systems, and operational workflows.
- Acts as a mentor and role model to provide excellent patient care and service and collegiality with Center teams.
- Ensure excellent patient service and effective resolution of patient concerns.
- Support efforts to enhance health equity and culturally competent care delivery.
- Ensure compliance with federal, state, and local healthcare regulations, including TJC, FTCA, OSHA, HIPAA, HRSA, CCBHC, SAMHSA, UDS/HEDIS, and other healthcare standards.
- Provide case conferencing and care coordination.
- Contribute to continuous readiness efforts for Joint Commission, FTCA, HEDIS, NOM’s and UDS and other quality measurement through survey preparation, mock tracers, and corrective action planning.
- Complete WellSpace Health Leadership training as DELTA, LEAD and other leadership training as needed.
- Respond to crisis calls and situations when escalated to leadership level.
- Provide technical assistance, training, and ongoing support to managers, staffing on program policies and procedures, including member enrollment, benefits, position descriptions, training requirements, service hours, retention strategies, program design and implementation, compliance requirements, prohibited activities, and other program-related areas as needed.
- Design and maintain organizational dashboards to monitor and improve LIA’s productivity and performance metrics.
- Develop and facilitate training programs, mentorship opportunities, and innovation initiatives that support employee development and organizational growth.
- Support the quarterly and annual performance review process for care stream support staff and providers including competencies for support staff and supporting clinician peer review chart audit and feedback.
- Lead cross-stream initiatives that enhance workflow consistency, accountability structures and interdepartmental collaboration.
- Represent WellSpace Health professionally and accurately in HRSA, SAMHSA, state and local forums focused on quality improvement and workforce development.
- Facilitation and/or attendance at appropriate meetings.
- Additional duties as assigned.

The work environment is characteristic of a healthcare clinic environment.
