The Role
The DH Case Manager supports the intermediate and long-term residential stability of residents in ASD's Destination Home program. This full-time position reports to the DH Housing Program Manager and plays a key role in delivering client-centered case management that upholds ASD's mission of dignity, confidentiality, and public trust.
What You'll Do
The DH Case Manager ensures the efficient delivery of quality, inclusive, and professional case management services through assessment, planning, service coordination, and ongoing monitoring. This role maintains accurate client records and works cross-functionally to support client success.

Provide assessment, planning, and case management services to residents, including intake calls, off-site interviews, and intake planning.

Develop and monitor individualized care and growth plans; assess client stability and adjust referrals to increase self-sufficiency.

Provide options and referrals related to substance use, mental health, and other community and government services (e.g., SSI, SSDI, Medicaid, Medicare, housing programs), and monitor referral completion.

Coordinate discharge planning and referrals to appropriate community agencies to support post-residency care.

Conduct community outreach and share information on agency procedures, health and welfare resources, and housing program success strategies.

Maintain accurate, up-to-date client documentation and statistical data in HMIS and other agency systems in a timely manner.

Collaborate across departments and attend case conferences and other meetings as directed to support client success.

Participate in agency fundraisers and respond to calls or emergencies as needed.

Qualifications
Required
-
BA/BS in social work, human services, or a related field; MSW, LCSW, or three years of relevant experience.

Proficient in English oral and written communication.

Access to reliable transportation.

Familiarity with databases, the internet, and Microsoft Word, Excel, and Outlook.

Ability to sit for long periods; lift up to 15 lbs.; and stand, stoop, bend, push, pull, and climb stairs.
Preferred
-
Experience in psychology or social services, particularly in HIV/AIDS care.

Experience providing case management to populations affected by mental illness, substance abuse, chronic health conditions, and/or homelessness.

Fluency in Spanish.
