Position Summary

As aPhysical Therapist (PT), you will evaluate and treat residents to improve mobility, strength, and overall function. You will work closely with an interdisciplinary team to deliver personalized, patient-centered care.

Key Responsibilities

• Performpatient evaluations and develop individualized treatment plans
• Provide therapy to improvemobility, strength, balance, and functional independence
• Monitor patient progress and adjust treatment plans as needed
• Maintain accurate and timelyclinical documentation
• Communicate effectively with physicians, residents, families, and care team members
• Participate incare planning and discharge planning
• Conductstaff education and in-service training
• SupervisePTAs, aides, and students as needed

Qualifications

• Degree inPhysical Therapy (Bachelor’s required; Master’s or Doctorate preferred)
• Active, unencumberedPhysical Therapist license in South Carolina (required)
• Experience in askilled nursing or rehab setting preferred
• Familiarity withPointClickCare (PCC) and Casamba preferred
• Strong clinical, communication, and leadership skills
• Commitment to patient-centered care
