NeighborHealth is seeking an experienced Prevention Screening and Referral (PSR) Coordinator to serve as a key operational and clinical leader within Project SHINE. This role is ideal for a professional with deep expertise in managing integrated prevention, screening, and referral programs, coordinating team schedules, maintaining regulatory compliance, and leading outreach initiatives for diverse populations.Schedule: Monday - Friday 8:00am - 5:00pm
What You'll Do
The Prevention Screening and Referral (PSR) Coordinator manages and coordinates DPH/OHA-funded prevention, screening, and referral programs, ensuring overall programmatic, operational, and service delivery compliance while supervising staff and providing direct patient services.
The successful candidate will be able to perform the following responsibilities:
- Coordinate and oversee Prevention Screening and Referral Services & activities in compliance with MA DPH/OHA requirements & standards.
- Ensure adequate coverage for all clinical sessions and hours of operations by appropriately coordinating staff schedules.
- Provide the prevention and screening team with supervision, support, training, and guidance.
- Maintain close communication with Department Manager & Director regarding PSR activities, initiatives, commitments, and work together to address programmatic or staffing challenges/barriers.
- Oversee adequate inventory of supplies, educational materials, necessary forms, and ensure timely, appropriate delivery of specimens.
- Adhere to DPH/OHA reporting requirements, maintaining accurate and organized documentation of testing data in EPIC and other database systems.
- Develop and update documentation tools, forms, and mechanisms to facilitate data collection.
- Work closely with PrEP Manager on effective and efficient PrEP navigation workflows and services.
- Work closely with PrEP Navigators to organize and coordinate in-reach/outreach efforts, specifically including health education and HIV counseling and testing at EBNHC for patients/staff and in the community.
- Participate and collaborate as part of the Quality Management and Improvement Team.
- Provide orientation & training to new Prevention staff; ensure training manual is assembled and updated.
- Review integrated HIV/STD/HEP C testing, counseling session content, documentation, specimen collection, policies & procedures, confidentiality, management of EPIC encounters, and provider communication.
- Ensure adherence to MA DPH/OHA specimen collection, handling, packaging, and submission by performing quality control reviews and properly training staff.
- Perform venipuncture, sample collections, specimen handling and processing, submission forms, and packaging and shipping process for all samples (collect and process blood and urine samples for testing).
- Interact and promote services across departments to increase and channel referrals through confidential testing programs.
- coordination and tracking of patients on injectable PrEP,
- support expansion of PSR services to primary care departments
- Oversee collaboration with universities/colleges  and work closely with designated Health Educator/Counselor around orientation, documentation, and training requirements.
- Collaboratively work with Program Manager on program planning and development (expansion toNeighborHealth departments, formal linkages with CBOs/ASOs for mobile unit services, staffing patterns, new initiatives).
- Provide input and collaborate with Program Manager on annual performance evaluations of PSR staff.
- Actively participate in the planning and coordination around the delivery of HIV positive results ensuring client and staff safety.
- Accompany Program Manager to MA DPH/OHA State Mandated Meetings and conferences; actively collaborate to meet contractual reporting deliverables & deadlines (Annual Work plan, Annual progress reports, HIV/STD Reporting, etc.).
- Review, download, and process all test results from Electronic Laboratory Reporting (ELR); enter/document all tests and procedures in EPIC; fill out and send new HIV Case Reports to the HIV/AIDS Surveillance Program.
- Collaborate effectively with Project Nurse to ensure individuals testing positive for HIV/STDs/HepC receive treatment and are linked to care; track positive patients referred to treatment or PCP follow-up.
- Report and coordinate partner notification for new HIV positive patients with the Disease Intervention Specialist program (DIS).
- Prepare for and actively participate in monthly calls with DPH/OHA contract manager; complete monthly reports in advance of monitoring calls.
- Participate in required departmental activities, events, and meetings (Huddle, Weekly PSR Meeting, Staff Meetings, Leadership Meetings, Joint Prevention & screening & clinical team meetings).
- Serve as the contact person for mobile unit sites; maintain working relationships with substance abuse treatment facilities, community-based agencies, and schools in CREW area and schedule prevention & education groups.
- Actively participate in funder monitoring site visits and address any recommendations and/or findings.
- Identify training needs for the prevention & screening team.
- Maintain flexibility to adapt to changes inorganization & department needs, scheduling, and job content.

What You’ll Bring:
- Bachelor’s degree (BA/BS) preferred.
- Community Health Worker Certificate and/or experience providing community health work to culturally, economically, and socially diverse communities, including LGBTQ+ community.
- Proven experience working in a team environment and leading program initiatives.
- Strong interpersonal and communication skills, with the ability to build rapport across diverse teams.
- Good assessment, crisis intervention, and counseling skills.
- Good understanding of substance abuse dynamics and cultural issues.
- Bilingual and bicultural strongly preferred.
- Exceptional problem-solving and conflict-resolution abilities.
- Ability to handle sensitive and confidential matters with professionalism and discretion.
- Proficiency in Microsoft Office Suite (Word, Access) and Electronic Health Records systems (EPIC).
- CPR Certification required
Salary: Starting at $24/hr up to $36/hr with relevant experience
