Join us on our journey to empower over a million people to change their behaviours and thrive!
At Thrive Tribe we deliver award-winning healthy lifestyle and wellbeing programmes that make people happier and healthier!
We’re looking for 2 Community Stop Smoking Advisors (one role will have a Children and Young People focus) to join our Smokefree Island service.
This is primarily a face-to-face, community-based role, focused on delivering high-quality smoking cessation support to people across the Isle of Wight. Advisors will manage a caseload of clients through their quit journey, delivering one-to-one and group support in community settings and clinics, with the ability to provide remote support where appropriate.
You’ll be accountable for your performance across key stages of the client journey and will work towards agreed individual and service targets. This includes delivering effective interventions, maintaining accurate and timely client records, supporting clients to remain engaged with the programme and ensuring outcomes are recorded appropriately.
The role also extends beyond your client appointments. You’ll play an active part in increasing awareness and uptake of the service within your local area, attending community engagement events and activities, building local relationships and helping to generate new referrals.
Like all roles at Thrive Tribe, you can expect your time at work to be fun, varied and challenging. We don’t like to stick to a firm JD as we know from experience that things change throughout the lifetime of a role, and it gives our people the chance to play to their strengths.
A typical day as a Community Stop Smoking Advisor might include:
• Delivering evidence-based, predominantly face-to-face interventions focused on smoking cessation, behaviour change and psychological support, working with clients on a one-to-one and group basis across community settings and clinics.
• Managing your client caseload effectively and taking responsibility for supporting people throughout their quit journey, including appropriate follow-up and re-engagement where required.
• Maintaining accurate, complete and timely client records, including appointment notes, outcomes and follow-up actions, so that clients receive the right support at the right time and their progress can be effectively monitored.
• Working towards agreed individual and service targets across key stages of the client journey, taking ownership of your performance and contributing to the overall success of the service.
• Supporting people to stop using vape/e-cigarette products where appropriate.
• Taking an active role in generating referrals within your local area, attending community engagement events and activities, promoting the service and developing relationships with local partners and communities.
• Using non-client-facing time productively to support wider service priorities and activities.
• Working with partner organisations and colleagues across the Isle of Wight to improve service uptake, reach priority communities and support achievement of service targets.
• Ensuring an excellent and safe experience for clients by following our policies and procedures.
• Giving due consideration to our Safeguarding, Information Governance and Clinical Governance policies and procedures. To ensure safer recruitment and safeguarding for all, we adopt pre-employment processes including DBS checks.
• Working with colleagues across all our Thrive Tribe services to contribute to new projects and service developments – we’re always looking to share knowledge and resources!
• Helping us maintain the ethos of Thrive Tribe by driving our organisational culture forward across our services and using every opportunity to embed our values!
Our Children and Young People (CYP) role also focuses on:
1.Deliver Stop Smoking and Vaping Cessation Support: Provide one-to-one and group-based, evidence-based support to children and young people, including comprehensive assessments, personalised quit plans, appropriate NRT advice, health education and relapse prevention.
2.Develop Outreach and Raise Awareness: Plan and deliver age-appropriate educational sessions, school visits, community events and online campaigns to promote the service and raise awareness of the harms of smoking, vaping and nicotine use.
3.Build Partnerships and Facilitate Referrals: Establish and maintain effective relationships with schools, youth organisations, healthcare professionals and community partners to co-produce services, improve referrals and increase service uptake.
4.Promote Health Equity and Collaborative Working: Target communities experiencing health inequalities and higher smoking prevalence, working collaboratively with multi-agency partners to provide holistic support and improve outcomes for young people.
5.Support Service Development and Best Practice: Apply the latest evidence-based interventions, contribute to professional training and local strategies, educate key stakeholders, and support the achievement of service targets and improvements.
