Pay Range: $29/hour.  Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).
 
Key Responsibilities:
  • Connect members to known community and care resources to facilitate their care management journey.
  • Support the coordination of community outreach resources and promote awareness of available services.
  • Assist members with inquiries regarding community and care resources.
  • Document and maintain community resources to comply with health plan standards and policies.
  • Collaborate with clinical teams, providing outreach, community education, and member support.
  • Conduct non-clinical health assessments to refer members to appropriate services.
  • Perform telephonic and in-person outreach to engage hard-to-reach individuals and families.
  • Visit homes and community organizations as needed.
  • Understand and address Social Determinants of Health (SDOH) barriers.
  • Comply with all policies and standards while performing other assigned duties.
Key Requirements and Technology Experience:
  • High School diploma or GED.
  • 1 – 2 years of related experience.
