Community Outreach Specialist

Pay: $75,000 per year base salary plus commission
Schedule: Full-time, direct hire
Where: Rosemead, California, out in the community with a home office nearby

The Opportunity

Seniors in the San Gabriel Valley deserve care that keeps them healthy and living at home. As a Community Outreach Specialist you are the face of a senior health program (PACE) in your neighborhood. You meet families, doctors, hospitals and community groups, earn their trust, and connect eligible older adults to the care they need. You are measured on referrals and enrollments, and you earn commission on top of your base.

How You Will Spend Your Week

• Out in the field at senior centers, health fairs, churches and community events
• Visiting physician offices, hospital discharge planners and skilled nursing facilities to build a steady referral flow
• Following up with interested families and setting home visits
• Planning your monthly outreach calendar with your director and tracking results in Salesforce and Athena

You Will Thrive Here If You

• Love meeting new people and opening doors yourself
• Have 2+ years in consumer sales, outreach or community marketing
• Are comfortable with a flexible schedule that sometimes includes evenings and weekends
• Hold a high school diploma or equivalent

Big Plus - Not Required

• Speak Mandarin (highly preferred) or Spanish
• Have worked in healthcare, senior services or community health
• Know your way around a CRM
