PURPOSE OF POSITION: Community Coordinator (Georgia THRIVe)

The Georgia THRIVe Community Coordinator is a key Infant Toddler Court Program role within Easter Seals North Georgia that collaborates  with the court, child welfare, and early childhood systems to address systematic gaps for families with infant and toddlers involved, or at risk of  nvolvement, with child welfare. The Community Coordinator will work at the family and community level to support the implementation of the  Georgia THRIVe Infant Toddler Court Program using the Safe Babies Approach. This unique role addresses systemic gaps by:

    · Building linkages across systems so that services are better aligned and coordinated

    · Providing a consistent and strong voice for the urgent developmental needs of infants and toddlers, including the critical role of early              caregiver–child relationships

    · Elevating the parental voice throughout the child welfare process

Note: This role does not involve case management or home visiting. The Community Coordinator serves as both a systems convener and family support coordinator, working to strengthen cross-sector collaboration, address service gaps, elevate the voices of parents and caregivers, and promote the healthy development and well-being of young children.

 

ESSENTIAL DUTIES AND RESPONSIBILITIES (include but are not limited to):

BEHAVIOR STANDARD

* Maintains a positive work atmosphere by acting and communicating in a manner enabling strong relationships with customers, clients, co-workers, and management.

 

ESSENTIAL FUNCTIONS

* Convene and maintain partnerships with community interest holders to support full implementation of the Safe Babies approach and Georgia THRIVe model and improve the coordination, alignment, and integration of services for infants, toddlers, and families. * Coordinate collaboration among court, child welfare, and early childhood systems partners * Facilitate monthly multidisciplinary Family THRIVe Meetings, Site Implementation Team (SIT) meetings, and Active Community Team (ACT) meetings using a strengths-based, solution-focused, and neutral approach that promotes shared decision-making and meaningful participation from all team members.

* Meet the ongoing educational and training needs of local partners implementing the Georgia THRIVe Infant Toddler Court Program * Promote systems change for the prenatal to three populations and their families through cross-sector collaboration * Enroll referred families into the Gwinnett THRIVe Program * Facilitate service coordination for program participants * Meet with enrolled program families weekly to assist with achieving their case plan goals and maintain regular contact *Elevate and amplify parent and caregiver voices throughout the child welfare process.

Attend court hearings *Schedule and maintain current calendars for all family and program meetings and court hearings

Be knowledgeable about existing and emerging community resources, identify gaps in services, and collaborate with partners to develop solutions that strengthen local systems of care.

Utilize program and community data to identify barriers, inform strategic planning, and support continuous quality improvement efforts.

Ensure documentation, reports, and records are completed and submitted according to requirements * Participate in statewide and nationwide Community of Practice calls with other Community Coordinators * Participate in regular meetings with the Georgia THRIVe state leadership team * Travel within the county for court, community resources, and required program trainings and conferences

 

PHYSICAL JOB REQUIREMENTS

* The ability to stand for sustained periods of time.

* The ability to walk for sustained periods of time including responding quickly to safety concerns

* The ability to use hands and fingers to feel, operate a computer, and perform picking, pinching and whole hand or arm handling tasks.

* The ability to climb stairs.

* The ability to stoop, kneel, crouch, sit on the floor, or crawl as needed.

* The ability to reach, extending arms in any direction.

* The ability to push or pull objects weighing up to 25 pounds.

* The ability to talk in order to accurately exchange information.

* The ability to hear in order to accurately exchange information.

* The ability to see clearly in order to maintain child safety and supervision, use a computer, and for extensive reading and writing.

* The ability to work in indoor conditions.

The ability to travel regularly within the county and occasionally throughout the state for court, meetings, training, conferences, and community engagement activities.

 

Qualifications

 

QUALIFICATIONS: Community Coordinator (Georgia THRIVe)

* Ability to work collaboratively with a multidisciplinary group across agencies and with a team across multiple agencies throughout the county and the state

* Bachelor Degree in social work, child development, sociology or relative field, preferred.

* Familiarity with Gwinnett County community and local resources including early intervention, medical care, education, mental health, and    behavioral health

* Understanding of issues related to child abuse and neglect, intimate partner violence, substance use disorders, mental illness, and trauma

* Experience providing trauma-informed support to families

* Highly collaborative management style

* Excellent organizational and project management skillsProficient in Microsoft Word, Teams, Zoom, WebEX and Outlook and able to learn  new technologies and databases as required

* Ability to work independently and as part of a team

* Performs other duties as assigned.
