Position Summary

As aLicensed Practical Nurse (LPN), you will provide direct nursing care while ensuring residents receive safe, compassionate, and high-quality treatment. You will work collaboratively with the care team to maintain a positive and supportive environment.

Key Responsibilities

• Deliverdirect patient care in accordance with individualized care plans
• Administer medications and treatments as prescribed
• Monitor resident conditions and report changes to the clinical team
• Support and superviseCNAs to ensure quality care delivery
• Maintain accurate and timelyclinical documentation
• Promote a safe, clean, and respectful environment
• Collaborate with interdisciplinary team members

Qualifications

• Active, unencumberedLPN license in South Carolina (required)
• Current CPR certification required
• Must maintain all continuing education and remain in good standing with the State Board of Nursing
• Experience withPointClickCare (PCC) preferred
• Strong clinical and communication skills
• Compassion for an underserved population
