Pay Range:$25/hr - $27/hour. Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).
Key Responsibilities:
- Assess member needs and assist in developing individualized care and service plans aligned with their goals and preferences
- Connect members and families with community resources, including housing, employment, independent living, foster care, and other social services
- Help members determine eligibility for available programs, obtain required documentation, and complete applications
- Coordinate referrals to appropriate healthcare providers and community service organizations
- Conduct outreach and follow-up with members, caregivers, and providers to ensure timely access to services
- Monitor service delivery and document outreach activities, assessments, and care plans in an electronic health record system
- Collaborate with community organizations, government agencies, healthcare providers, and other stakeholders to address member needs
- Provide education regarding available benefits, services, and community resources
- Maintain accurate, complete, and up-to-date resource information and case documentation
- Support the development of resource-service procedures, educational materials, and training programs
- Conduct telephone and in-person outreach, including home visits and community or agency meetings
- Ensure compliance with established care management policies, procedures, and documentation standards
- Perform additional duties as required
Key Requirements and Technology Experience:Â
- 2-4 years of relevant professional experience
- Experience in child welfare or related community-based services
- Experience connecting children and families with community resources such as housing, employment, foster care, independent living, or social services
- Proficiency in Microsoft Office, including Outlook, Word, and Excel
- Valid driver’s license, as field visits are required
- Ability to work remotely while traveling into the community for scheduled visits and meetings
- Bachelor’s degree
