MHM Solutions, a Centurion company, is proud to be the provider of comprehensive health services and staffing solutions to the Commonwealth of Pennsylvania.  We are currently seeking a full time Clinical Director to join our team atthe Office of Developmental Programs located inPittsburgh, Pennsylvania.Â
Shift: Hybrid work – 2 days onsite minimum in Pittsburgh; potential for other travelÂ
Responsibilities:Â
•  Act as a resource and provide TA to provider/AE’s/Community Mental Health/County Mental Health related to ID/DD and Co-occurring MH/Behavioral Phenotypes/Problematic Sexual behavior as requested-whether called with questions or requested for specific TA. Advise on cases according to ODP policy and best practices work with the program rep for a specific county when doing so.
•   Participate and lead transition planning for individual moving to the community.Â
•   Review behavioral incidents monthly (due by the 15th of each month). Utilizing HCSIS reports, identify individuals who may be at risk and review their ISP, SC notes, previous incidents, etc.  Of this review, create a monthly report to be shared with DD lead for DD QM measures and the regional office.
•   Weekly participation in diversion calls.
•   Complete Higher Level Reviews as needed when a person with ID is at risk of placement in a State.Â
•   Regional PPRT lead-Triage PPRT referrals to assure they are appropriate and meet criteria. Maintain current knowledge of community resources across the region and state to connect regional office/AE’s with appropriate resources as needed.
•   Co-Lead regional PPC efforts.
•   Co-facilitate and organize Statewide PPC efforts. Participate in monthly PPC leadership call and quarterly meetings.
•   Work with Risk Management and Quality Management as needed and develop strategies with risk management to mitigate risk associated with DD.
•   Work with statewide DD team. Weekly team call for support and to identify statewide needs and activities. Projects as identified/assigned.
•   Act as SME on DD and provide information to regional office as needed/requested. Activities as identified/requested by RPM
•   Design’/provide training as needed per TA or as requested by various groups. All curricula should be submitted for DHS approval.
•   Maintain contact with various MH resources and local BHMCO’s. Maintain contact list and interact as needed.
•   HCQU backup-attend MOC and Director meetings as needed in coordination with regional nurse. Also provide expertise to HCQU’s. Maintain understanding of each HCQU initiatives and activities in order to advise and refer as needed on individual needs
•   Weekly incident closure of behavioral incidents as identified by RPM.Â
•   Primary contact for Dual Diagnosis Treatment Teams-provide TA as requested and support requested in specific cases. Participate in 30 day calls/meetings when needed
•   Remain up to date on all applicable research and maintain files for sharing as needed related to best practices.
•   Inform policy as requested based on best practices and trends.
•   Maintain regular contact and support to the field in order to understand current struggles/needs and provide support as needed.
•   Participate in CSP’s and PCP’s as needed in high level cases or those where teams/SMH are struggling.
•   Assist provider community in developing capacity/training/support strategies related to DD.
•   Assist Risk Management/Quality Management in efforts toward restraint reduction/elimination
•   Various tasks as assigned.
Qualifications
- Graduate degree in Human Services (psychology, nursing, social work or a related field), and six (6) years clinical experience in working with individuals with intellectual disabilities (ID) with comorbid mental illness (MI), challenging behavior and problematic sexual behavior.
- Ability to orally communicate effectively in English in all levels of group and individual contacts/setting.
- Ability to communicate effectively in writing, using the English language, including computer based communication using e-mail, word processing, and data based systems.
- Knowledge of Positive Practices/Approaches and the Recovery Model frameworks in Pennsylvania.
- Experience in working with multiple stakeholders and facilitating collaborative practices/supports for persons with dual diagnosis (ID/MI)
- (Provider, Administrative Entity, Health Care Quality Unit (HCQU), etc.).
- Experience in analyzing quality/risk management data to identify opportunities/strategies for system improvement. Â
- Intermediate computer skills including the use of Microsoft Office software and web based applications.Â
