- Own your territory and your number. Meet or beat monthly start-of-care and revenue targets. Know exactly where you stand, every day.
- Hunt constantly. Identify, prospect, and open doors with discharge planners, case managers, SNF and rehab teams, senior living directors, geriatric care managers, elder law attorneys, trust officers, and financial advisors. There is always a next account, and you are always working it.
- Live in the field. Build a weekly route plan and run it. Face-to-face visits, lunch-and-learns, in-services, and drop-ins are how this job gets done. Desk time is the exception.
- Convert, do not just connect. Turn relationships into referrals and referrals into starts. Move fast on every inquiry, in person, by phone, or web. Speed to response wins families.
- Run the assessment and close. Meet families in homes, hospitals, and facilities. Understand what they need, walk them through the service agreement including payment and billing terms, and guide them to a clear decision.
- Take share. Know which competitors are in your accounts, why they are getting referrals, and your plan to displace them.
- Work your pipeline with discipline. Rank accounts by referral potential, log every visit and opportunity in HubSpot the same day, and use your data to decide where your next hour goes.
- Be where the decision-makers are. Represent at networking groups, industry events, association meetings, and community committees, and walk out with meetings booked.
- Grow what you win. Stay close to active referral cases through family meetings, home visits, and partner check-ins so your sources keep sending business.
- Answer when it counts. Hospital discharges and family crises do not keep office hours. Be available to referral partners, prospects, and clients on evenings and weekends as needed.
- Hand off clean. Partner tightly with operations and finance so every client you bring in gets a smooth start and an accurate account setup, especially when the referral comes through a legal or financial advisor.
- Play by the rules. Maintain working knowledge of obligations as a licensed home care agency in NY, NJ, and CT, including obligations to clients and caregivers, and protect client privacy at all times.

WHAT YOU BRING
- A proven track record of hitting or beating quota in a field sales or business development role. Experience selling into healthcare, home care, home health, hospice, senior living, medical device, or pharma is a strong plus.
- A clinical or social work background is a plus: Registered Nurse with a bachelor's degree, or licensed social worker, in NY, NJ, or CT.
- Ability to spend roughly 90% of your time in the field within your assigned territory; current driver's license and proof of insurance.
- CRM discipline. Same-day, accurate documentation in HubSpot is non-negotiable.
- Ability to use territory data, pipeline metrics, and market intelligence to decide where to spend your time.
- Skill in consultative, emotionally complex sales with multiple decision-makers: adult children, spouses, advisors, and clinicians.
- Ownership of the admin that keeps a territory running: account plans, routing, expense reporting, and collateral.
- Comfort with Google Workspace, video conferencing, and LinkedIn and social selling.
- Integrity, attention to detail, and strong emotional intelligence.
