Registered Nurse - Hospice Case Manager

Looking for a rewarding nursing career? We are seeking a FT Day RN - Case Manager to provide routine home care visits and complete admissions while delivering compassionate, high-quality care!

• FT Day 8a-5p

• Will manage a case load of patients

• RN MO or KS or Compact required

• BLS required

• Flexibility a must

The Opportunity

This position will manage a case load of patients collaborating with the interdisciplinary team. Accountable for utilizing the nursing process. Is responsible for delegating patient care tasks and for supervision of other patient care staff. Is responsible for coordinating the care for a group of patients and consulting with other members of the health care team when indicated. Provides nursing care in compliance with the Nurse Practice Act.
