Title: Child & Family Case Manager

Location:Child & Family - 1954 SE 182nd Ave, Portland, OR 97233

Schedule: This is an in person position, Monday through Friday, 8:30am - 5:00pm; Occasional evenings and weekends required depending on program needs.

Wage Range:$26.00 – $29.00 hourly, non-exempt

Position Overview:

The Child & Family Case Manager provides case management, skills training, systems navigation, and supportive services to children, youth, and families experiencing behavioral health needs and complex needs. This position assists youth and families in accessing mental health services, community resources, and other supports while helping reduce barriers to care. The Case Manager helps youth and families navigate service systems, build skills, access resources, and remain engaged in recommended services and supports.

 

What you will do:

  • Provide case management services to children, youth, and families receiving services through the Child & Family Program.

  • Assist youth and families in identifying needs and accessing mental health services, educational supports, social services, and community resources.

  • Provide systems navigation, referrals, and warm hand offs to help youth and families access and coordinate services across behavioral health, educational, social service, and community systems.

  • Provide skills training and supportive services that help youth and families successfully engage in treatment, services, and community-based supports.

  • Provide support to youth and families experiencing behavioral health crises and assist with accessing appropriate crisis, behavioral health, and community resources.

  • Assist with transportation coordination and other supports that reduce barriers to accessing services and treatment including transporting clients to appointments as needed.

  • Maintain regular contact with youth and families to support engagement in services, identify barriers, and provide ongoing support.

  • Track client engagement, referrals, and service activities; maintain accurate documentation in accordance with agency, funding, and regulatory requirements.

  • Provides trainings and education as well as social, cultural, and/or therapeutic groups to promote mental health awareness and wellness.

 

Qualifications

We would like to hear from people that have:

  • Either High School Diploma or General Educational Development (GED) certification required.

  • Current Qualified Mental Health Associate (QMHA) or Qualified Mental Health Professional (QMHP) certification through the Mental Health & Addiction Certification Board of Oregon (MHACBO).

  • Valid driver's license, proof of auto insurance, access to reliable transportation, and ability to meet NARA NW's driver eligibility requirements.

  • Proficiency with basic computer applications and electronic documentation systems.

  • Must pass a pre-employment or for cause drug tests.

  • Must pass criminal background and DMV checks.

Preferred Qualifications:

  • Associate's degree in Human Services, Social Work, Psychology, Behavioral Health, Education, or a related field.

  • One (1) year of experience providing case management, skills training, behavioral health support services, or systems navigation services.

  • Experience working with children, youth, and families.

  • Experience working within the Native American/Alaska Native community.

  • Knowledge of behavioral health services, educational systems, social service programs, and community resources.
