Job Responsibilities

• Assist patients with bathing, dressing, grooming, and personal hygiene.
• Provide assistance with mobility, transfers, and ambulation.
• Assist with activities of daily living (ADLs).
• Provide companionship and emotional support.
• Assist with meal preparation and light housekeeping as authorized.
• Observe and report changes in patients' conditions.
• Document care provided accurately and promptly.
• Follow established care plans and infection prevention procedures.
• Communicate professionally with patients, families, and nursing supervisors.

Qualifications

• Current Texas CNA certification preferred; required for assignments designated for CNAs.
• Home health, home care, or long-term care experience preferred.
• Reliable transportation and willingness to travel within your selected service area.
• Strong communication and interpersonal skills.
• Compassionate, dependable, and professional.
• Ability to work independently in patients' homes.
• Ability to complete required documentation electronically.
• Must meet applicable background screening and agency hiring requirements.
