GENERAL SUMMARY:
The Case Manager Assistant provides essential support to Case Managers by assisting with concrete patient-related tasks and various clinical interventions. This position serves patients, families, and others who are requesting assistance from the Case Management & Social Work Services Department but who are not currently receiving services from another staff member. The Case Manager Assistant supports the department by performing a variety of office-related responsibilities, including assisting with special medication fund programs, updating resource materials, and issuing Medicare Important Messages to patients as needed.
EDUCATION/EXPERIENCE REQUIRED:
- High School diploma or G.E.D. equivalent
- Associate's degree preferred or equivalent healthcare experience
