The OrganizationÂ
Project Guardianship (PG) is a 501c3 nonprofit organization providing legal guardianship services for older adults and people living with disabilities and mental illness in New York who have no family or friends able to assist them. PG’s team approach provides guardianship clients with services that allow them to be as independent and in the least restrictive setting possible. Since 2005, PG has served hundreds of New Yorkers and has become a leading voice in a national, interdisciplinary effort to re-envision and strengthen guardianship policy. More recently, PG created training and advocacy initiatives to help improve older adult and disability services throughout New York and to ensure that older residents have access to good guardianship and other decision-making support services, regardless of their ability to pay.Â
The PositionÂ
Â
As a member of the case management team the Case Manager will collaborate closely with other PG staff, various organizations and institutions to deliver effective case management services to our clients. As a case manager you will provide care coordination services and advocacy to ensure clients’ well-being, safety, and the greatest degree of independence possible, within the least restrictive environment.Â
Primary Responsibilities:Â
1. Care coordination and case management:Â
Care oversight for a caseload of 20-25 clients who are aging or living with a disabilityÂ
Client initial intake, and monthly and annual assessmentsÂ
Regular monitoring of client’s condition and services through monthly in-person or virtual visits, and more often via calls, emails, and/or texts, if neededÂ
Coordination and participation in medical and end-of-life decisions (in consultation with legal staff and medical professionals)Â
Communication and advocacy for our clients with other agencies and institutionsÂ
Close work and communication with clients’ families and loved onesÂ
Active participation in team meetings and case reviewsÂ
Initiation and coordination of pre-paid funeral arrangementsÂ
Keeping close communication with medical and home care providersÂ
Arranging client’s home cleaning and extermination, if neededÂ
2. Case Notes/ Reports and other documentation:Â
Keeping timely and detailed case notes, records of communications with partiesÂ
Timely completion of Initial, annual, and final accounting reportsÂ
Administrative documentation, and reports to grants/funding programsÂ
Organizing/scheduling work and assignments in your job Outlook calendarÂ
Updating and maintaining clients’ electronic files and records Â
3. Public and private benefits, assistance programs:Â
Screening clients for government and private benefitsÂ
Gathering needed information and completing and submitting applications and recertificationsÂ
Applying for and maintaining clients' medical insurancesÂ
4. Travelling throughout the five boroughs of NYC via public transportation to see clients, their families and other professionals. Travel to other regional locations may be required. Â

Education and Experience:Â
While a Bachelor's and/or Master's Degree in Social Work or related field is preferred, relevant experience and work history may be accepted in lieu of a degree.

CompensationÂ
$50,000 - $60,000 annuallyÂ

This role is hybrid; the employee will be required to work from our office in Dumbo, Brooklyn 1 to 2 times per week.
