Diabetes Care Clinic helps patients managecondition andenabling them to lead the lives they want to lead. The program Plantar Fasciitis (PF) preventive and maintenance services foradult suffering from diabetes.Â
Â
Under limited supervision,provides professional medical nutrition therapy to groups and individuals of all ages andabilities, in a variety of settings in the outpatient nutrition service area. Follows established standards of practiceand guidelines defined by the Academy of Nutrition and Dietetics, USDA, American Diabetes Association, and otherevidenced based resources for the assessment of patients, andsubsequent planning, implementation, evaluation ofnutrition education. Services alsoincludes continuum of care for out-patients requiring medical nutrition therapy.Works with an established multidisciplinary team of professionals to provide quality nutrition education servicesincluding all department services. Also providessupport the outpatient diabetes education services as needed.Â

Job Description:Â
Performs and documents a complete and comprehensive medical nutrition therapy planin accordance with scopeof practice, policies and procedures, national standards of care, as well as consideration of the needs, barriers, andskills/abilities/resources/support system of each patient service in theoutpatient setting. Â
Coordinates the nutritional plan of care for outpatients as part of the diabetes educationprogram, and supportsthe overall established diabetes educational curriculum.Â
Provides counseling in an effective and compassionate way, using active listening techniques and professionalbehavior, individualizing the care of each patient, setting mutually agreed upon goals, performing follow upactivities.Â
Works with RNs and all members of the team in the department to provide individual and group classes for alltypes of diabetes related services, includingDiabetes Prevention Program.Â
Crosstrained to provide diabetes education services asneed arises.Â
Accountable for measurements of effective and successful education to patients through measured goals and outcome reports, audits, and satisfaction survey/feedback.Â

Job Requirements:Â
Bachelor’s DegreeÂ
Current registration or registration-eligible with the Commission on Dietetic Registration. Those registration-eligible mustbecome registered within 6 months of date of hire unless extension grantedÂ
Licensed Dietitian in Kentucky or licensed-eligible after receiving CDR registration. Those licensed-eligible must becomelicensed in KY within 6 months after receiving CDR registration unless extension granted.Â
Preferred Requirements:Â
