Pay Range: $23 /hour.  Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).

Key Responsibilities:
  • Perform member outreach telephonically or through home visits to engage members and discuss care/service plans.
  • Coordinate care activities based on the care plan/service plan and collaborate with healthcare and community providers.
  • Serve as support for various member and/or provider inquiries related to care plans.
  • Document and maintain member records in accordance with state and regulatory requirements.
  • Identify needs and make referrals to Care Managers, community-based organizations, and Disease Managers.
  • Provide education on benefits and resources available to members.
  • Comply with all policies and standards while performing other duties as assigned.
Key Requirements and Technology Experience:
  • High School diploma or GED.
  • 1–2 years of relevant experience in care coordination, managed care, health plan member services, patient support, or case management.
  • Strong communication, attention to detail, multitasking, and documentation skills are essential.
