Program Summary:

Through a collaborative, non-traditional team approach, FSPW partners with youth and their families to build on their strengths and develop comprehensive service plans that may include evidence-based interventions, care coordination, and natural community supports. Services are flexible and accessible, delivered in homes, schools, clinical sites, and community settings to meet each family’s unique needs.
