Overview
We are seeking a dedicated Care coachto support Medicaid members through comprehensive care coordination, assessment, and case management services. This field-based role is responsible for visiting members in their homes, assisted living facilities, long-term care settings, and other community locations to assess needs, develop care plans, coordinate services, and help members achieve optimal health outcomes.The ideal candidate has experience working with healthcare or human services populations, strong case management skills, and a passion for improving the lives of vulnerable individuals through personalized care and community resource coordination.
Location Requirement: Must reside in Nassau or Duval County, Florida
Travel Requirement: Approximately 75-90% local travel within the assigned service area
Responsibilities
- Conduct in-home, assisted living, and long-term care facility visits with Medicaid members
- Assess member needs and develop individualized care plans
- Coordinate healthcare and community-based services to support member goals
- Monitor and evaluate care plans to ensure desired outcomes are achieved
- Ensure members receive services in the least restrictive and most appropriate setting
- Identify barriers to care and help members access needed resources
- Collaborate with providers, caregivers, community agencies, and interdisciplinary care teams
- Guide members and families toward appropriate healthcare and social service resources
- Maintain accurate case documentation and care management records
- Facilitate communication between healthcare providers, payer sources, and support services
- Support members in achieving and maintaining optimal health and well-being
- Manage multiple cases while maintaining compliance with program requirements and quality standards
Required Qualifications
- Must reside in Nassau County or Duval County, Florida
- Bachelor's degree in a health or human services-related field
- Minimum 2 years of related experience in care management, case management, healthcare, social services, or a related field
- Knowledge of Microsoft Word and Excel
- Strong verbal and written communication skills
- Strong interpersonal and relationship-building abilities
- Ability to use electronic information systems and software applications
- Valid driver's license
- Personal vehicle with required automobile liability insurance coverage
- Ability to travel extensively throughout the assigned territory
- Ability to successfully complete required Tuberculosis (TB) screening
- Commitment to delivering exceptional member experiences and quality care
Preferred Qualifications
- Experience working with Medicare and/or Medicaid populations
- Experience with electronic case note documentation
- Experience documenting within multiple healthcare systems and applications
- Experience with health coaching, wellness promotion, or disease management
- Knowledge of community health resources and social service agencies
- Experience coordinating care for complex or vulnerable populations
