LVN Supervisor

Sunnyvale Post-Acute is seeking a dedicated and leadership-minded Licensed Vocational Nurse (LVN) Supervisor to join our premier clinical team. In this role, you will oversee daily nursing operations, mentor staff, and ensure our residents receive the highest standard of compassionate, regulatory-compliant care.

Position Overview

• Job Title: LVN Supervisor
• Location: Sunnyvale, CA
• Compensation: $50.00 – $55.00 per hour (Depending on Experience)
• Job Type: Full-time / Shift-based
• Reports To: Director of Nursing (DON) / Registered Nurse (RN) Daily Supervisor

Key Responsibilities

• Clinical Oversight: Manage and monitor the daily nursing activities on your assigned unit or shift to maintain excellent patient care standards.
• Staff Leadership: Supervise, mentor, and delegate tasks to floor LVNs and Certified Nursing Assistants (CNAs), promoting a collaborative team environment.
• Resident Care: Administer medications, manage treatments, perform wound care, and assess changes in resident conditions.
• Compliance & Documentation: Ensure strict adherence to state, federal, and facility healthcare regulations. Maintain meticulous electronic health records (EHR).
• Admissions & Discharges: Coordinate with the interdisciplinary team to facilitate smooth resident transitions and care plans.

Qualifications & Skills

• License: Active, unrestricted California Licensed Vocational Nurse (LVN) License.
• Experience: Minimum of 1–2 years of nursing experience in skilled nursing, post-acute, or long-term care settings. Prior supervisory or charge nurse experience is highly preferred.
• Certifications: Valid CPR/BLS certification.
• Leadership: Strong communication skills with the ability to confidently lead a nursing team, manage shifts, and handle high-stress situations calmly.
