Position Summary |
The Manager of Bereavement and Community Grief is responsible for the day-to-day operations and delivery of bereavement and community grief services, including but not limited to: coordination and supervision of bereavement and community grief counselors and bereavement program in support of patient family members, and development and management of grief programs to support the needs of our community. Under the direction of the Regional Director and as part of the interdisciplinary group the Lead Social Worker coordinates all social service interventions and referrals, including the needs assessment, supportive counseling, networking, resource procurement and crisis intervention. Contributes to the smooth functioning of services, attends the IDG meetings as needed and advocates for the patients and family needs with the team at large. |
Essential Functions |
- Manages the development and delivery of all bereavement and community service programs to effectively address the grief needs of patient family and community members
- Monitors, tracks and ensures compliance with Standards and Regulations (e.g.CAHPs,CoPS, NASW, and Florida Administrative Code) through chart audits, as well as review and monitoring of documentation.
- Assists in the development of systems/tools to evaluate and monitor the quality of bereavement and community grief services; participates in QI activities and incorporates findings into team activities, as needed.
- Oversees the development and revisions of bereavement and community grief policy and procedures.
- Evaluates skills, including clinical, and provides constructive feedback, coaching and counseling to all Bereavement and Community Grief staff; prepares and conducts performance appraisals
- Provides specialized training and preceptsbereavement outreach volunteers.
- Ensures relay of all high risk, complex or potential abuse/neglect related to Community Hospice patients/families to appropriate staff.
- Provides clinical supervision to license eligible staff, as requested.
- Develops and maintains professional relationships with other social service departments and health care providers.
- Supports professional growth of team members through academic and continuing education programs, as appropriate.
- Functions as a bereavement counselor for designated IDG teams.
- Oversees participation of Bereavement and Community Grief department in orientation of new staff
- Other duties and special projects as assigned.
- Coordinates census/ratio with Senior Clinical Manager and determines the best utilization of the personnel resources assigned to the teams.
- Provides clinical consultations to those on their teams on an as needed basis
- Develops and maintains relationships with area social workers, discharge planners and community staff in order to network and facilitate referrals of patients.
- Participates in sharing information with other professionals through participation in such local organizations as National Association of Social Workers ( NASW).
- Complies with (NASW) Code of ethics and standards for end of life care in delivering services to patients and families.
- Supervision / consultation for all PSP staff for complex cases including process and documentation questions
- Supervision for non MSW/ BSW staff for COP regulation chart audits, ride along, face to face meetings or phone calls
- assist with training
- Assure coverage for PSP staff for PTO
- Chart audits of PSP regulatory/agency compliance through documentation and care plan review though Suncoast patient records.
- Caseload management for PSP staff including assignments/reassignments as staff are laid off or territories shift
- Mentor Staff
- Conduct small group staff meetings
- Participate in development/ update of PSP guidelines, competencies and processes
- Provide education to new staff on changing or new policies and procedures
- Participate in screening and interview process of new employee candidates
- Develop on call calendar

Qualifications |

Knowledge and Critical Skills |
- Principles, practices and methods of social work
- Grief and loss and overall bereavement care provision that includes needs assessment, case management, crisis intervention, supportive counseling, advocacy.
- Interpersonal communication and family dynamics and administration.
- Planning, prioritizing, coordinating, directing varied and complex administrative functions.
- Effective communication and professional presentation, both verbally and in writing.
- Working with multi-disciplinary team.
- Self-motivating.
- Supervising, training and evaluating employees.
- Ability to work competently with high levels of stress.

Experience |
- Minimum five (5) years clinical practice/direct service experience with individuals and families in health care.
Minimum two (2) years supervisory and program management experience. |
License / Certification |
- Florida LCSW,LMFT, or LMHC required.
- State certification as a “Qualified Supervisor” for Chapter 491 preferred.
- Current State of Florida driver’s license with driving record acceptable to Community Hospice.

Education |
- Masters degree in social work,mental health counseling or marriage and family therapyfrom an accrediteduniversity.

Other Requirements |
- Must have medical clearance for communicable diseases before direct patient contact.
- Current immunizations including but not limited to TB, Hepatitis, and Flu.
