Pay Range: $120,000-$150,000 Per Year
Description:
• The Advanced Practice Practitioner (NP/PA) in Buffalo, NY is a full-time role involving direct patient care both in-person and via telehealth.
• The position requires a strong knowledge base in healthcare practices, documentation, and coding, with a focus on quality patient care.

Requirements:
• Master's degree in nursing or related science field
• Active and unrestricted NP/PA license in the state of hire
• 2 years of direct patient care and clinical experience
• Experience in community-based care and Medicare/Medicaid
• Excellent verbal, written, and presentation skills
• Strong critical thinking and communication skills
• Proficient in generic technology applications
Qualifications:
• Master's degree in nursing or related science field required
• Active NP/PA license required in the state of hire
• 2 years of experience treating complex chronic conditions
• Experience with Medicare and LTSS preferred
• Excellent communication and group facilitation skills
• Strong analytical and problem-solving skills
• Proficiency in generic technology tools
