Requirements:
• Master's degree in nursing or related science field.
• Active and unrestricted NP license.
• Minimum of 2 years of leadership and management experience in a clinical setting.
• Experience with Medicare/Medicaid is essential.
• 2 years of clinical experience with complex chronic conditions and high-needs patients.
• Proficiency in general technology applications.
• Excellent verbal, written, and presentation skills.
Qualifications:
• Strong leadership and team management abilities.
• Comprehensive understanding of Value-Based Care, documentation, and coding.
• Experience in community-based care preferred.
• Strong critical thinking and communication skills.
• Excellent computer skills, particularly in general technology applications.
• Good judgment and ethical standards.
• Desire to grow in a fast-paced organization.
