What You’ll Do- As anAdult Behavioral Health Home Coordinator, you will:

• Serve as a central point of coordination for members' behavioral and physical health services
• Collaborate with clients, their natural support, and community resources
• Help individuals achieve optimal wellness through personalized, integrated care plans

What You Need-

• A certification as aMental Health Rehabilitation Technician/Community (MHRT/C) (or provisional MHRT/C)
• Knowledge oflocal community resources
• Comfort withelectronic health records and standard computer applications
• Valid driver’s license and reliable transportation

What We Offer-

Starting Salary: $22.00 – $25.00 per hour plus potential formonthly incentives!

• $5,000 recruitment & retention bonus: $1,500 at end of orientation, $1,500 at successful completion of probationary period, and $2,000 at 1 year employment

Qualifications

Education & Experience Requirements:

• This position requires a Mental Health Rehabilitation Certificate (MHRT-C) or MHRT provisional certification. (A MHRT-1 does not qualify.) If you hold a bachelor's degree in a related or unrelated field, you may be provisionally qualified. • Previous professional experience in the behavioral health field.
• Experience using an Electronic Health Record (EHR) system preferred.
• Reliable transportation and a valid driver’s license are required.
