Home Care Eddy Visiting Nurse & Rehab Association is seeking a compassionate and motivated Medical Social Worker to join our dedicated Home Care team. Responsibilities

• Conduct comprehensive psychosocial assessments in patients' homes.
• Develop individualized plans of care that address patient and family needs.
• Connect patients and caregivers with community resources, support services, and benefit programs.
• Collaborate closely with nurses, therapists, physicians, and other members of the interdisciplinary care team.
• Participate in case conferences and care planning meetings.
• Advocate for patients and families to promote optimal health, safety, and quality of life.
• Provide emotional support, counseling, and education related to illness, adjustment, and caregiving challenges.

Requirements:

• Master's degree in social work is required.
•  NYS Social Work Licensure required.
• Previous experience in healthcare, home care, hospice, or a related healthcare setting preferred.
• Excellent communication, assessment, and care coordination skills.
• Valid New York State driver's license and reliable transportation required.

Pay Range: $30.00 - $43.08

Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
