Endocrinology Nurse Practitioner

Join a collaborative and patient-centered endocrinology team where you'll make a meaningful difference in the lives of patients living with diabetes, thyroid disorders, obesity, osteoporosis, and other complex endocrine conditions. As an Endocrinology Nurse Practitioner, you'll practice at the top of your license, leveraging advanced clinical expertise, innovative diabetes technologies, and evidence-based care to improve patient outcomes and quality of life.

This role offers the opportunity to build lasting relationships with patients, provide comprehensive chronic disease management, and work alongside experienced endocrinologists and multidisciplinary specialists in a supportive, team-oriented environment.

What You'll Do

Comprehensive Patient Assessment & Diagnosis

• Conduct thorough initial and follow-up assessments of patients' endocrine, metabolic, physical, and psychosocial health.
• Obtain detailed medical, medication, family, and social histories with a focus on endocrine conditions.
• Perform comprehensive physical examinations and identify endocrine-related health risks.
• Evaluate risk factors associated with diabetes, cardiovascular disease, thyroid disorders, obesity, osteoporosis, and other endocrine conditions.
• Assess treatment adherence, self-management capabilities, and barriers to successful outcomes.
• Develop differential diagnoses using clinical findings, laboratory results, and diagnostic studies.
• Monitor patient progress and treatment response through ongoing evaluation and follow-up.

Advanced Diagnostic Evaluation

• Order and interpret endocrine and metabolic laboratory studies and diagnostic testing.
• Analyze blood glucose records, continuous glucose monitoring (CGM) data, and insulin pump reports.
• Evaluate thyroid function studies, hormone panels, bone density testing, and other specialized diagnostics in collaboration with endocrinologists.
• Utilize evidence-based guidelines to guide clinical decision-making and disease management.

Endocrine Disease Management

• Develop individualized treatment plans for patients across the spectrum of endocrine and metabolic disorders.
• Prescribe, initiate, titrate, and monitor medications for diabetes, thyroid disease, obesity, osteoporosis, and related conditions.
• Manage advanced diabetes technologies, including CGMs and insulin pump therapy.
• Recommend lifestyle interventions, medical nutrition therapy, weight management strategies, and exercise programs.
• Monitor treatment effectiveness, medication tolerance, and clinical outcomes to optimize patient health.

Patient Education & Empowerment

• Deliver personalized education tailored to each patient's learning needs and goals.
• Provide diabetes self-management education and support.
• Educate patients on insulin administration, glucose monitoring, CGM use, insulin pump management, and medication adherence.
• Counsel patients and families on nutrition, weight management, physical activity, and long-term risk reduction.
• Foster patient engagement, shared decision-making, and confidence in managing chronic endocrine conditions.

Collaborative Care Coordination

• Partner closely with endocrinologists, primary care physicians, diabetes educators, dietitians, pharmacists, and other healthcare professionals.
• Coordinate specialty referrals and connect patients with valuable community resources.
• Participate in multidisciplinary care planning and case management.
• Ensure seamless care transitions across outpatient, inpatient, and specialty settings.
• Build strong relationships with patients, families, and healthcare team members through exceptional communication.

Specialized Clinical Expertise

Primary emphasis on diabetes management, including:

• Type 1 Diabetes
• Type 2 Diabetes
• Gestational Diabetes
• Prediabetes

Additional areas of care include:

• Thyroid disorders (hypothyroidism and hyperthyroidism)
• Obesity and metabolic syndrome
• Osteoporosis and metabolic bone disorders
• Polycystic Ovary Syndrome (PCOS)
• Hypogonadism
• Endocrine-related complications and disease progression management

Quality, Documentation & Professional Practice

• Maintain timely, accurate, and comprehensive electronic health record documentation.
• Participate in quality improvement initiatives and population health programs focused on diabetes and endocrine care.
• Support organizational goals related to patient outcomes, quality metrics, and evidence-based practice.
• Maintain compliance with regulatory, organizational, and professional standards.

Qualifications

• Master's or Doctoral degree in Nursing from an accredited program.
• Current Nurse Practitioner licensure and prescriptive authority.
• Board Certification as a Nurse Practitioner.
• Current certifications and continuing medical education (CME) as required by GCMB, hospital policy, and specialty practice standards.
• Ability to meet physician/APP collaboration requirements as outlined by GCMB and CMS regulations.
• Previous endocrinology, diabetes management, or related specialty experience preferred.
