Summary
Position will be for Discharge Planning.Job Description:
Case management services are designed toassist patient and family members with continuing care arrangements that may be necessary following discharge from the hospital. Case managers work with physicians, nurses, physicaltherapists and other members of the health care team toidentify and coordinate alternative care or services that willassist in recovery.
The Case Manager strives for excellence in direct patient care by building a collaborative caring workenvironment coordinating care and information across the various disciplines focused on patient outcomesand wellness. The Case Managerassesses plans and facilitates options and services which best meet thepatients’ healthcare needs in the continuum of care. The Case Manager formulates goals with the patientand their families. The Case Manageradvocates for the patient through communication and evaluation ofavailable resources to promotequality cost-effective and safe outcomes. The Case Manager incorporatesstandards of care for Case Management including identification/selection of patients for case managementservices, problem identification, planning, monitoring, evaluating, transition of care and outcomes. TheCase Manager implements practice based on current research and evidence based practice and ensuresadherence to hospital and department policies, regulatory standards, and guidelines. The Case Managersupports an environment of caring focused on the patient’s healing and wellness.

Job Description:
Identifies and makesappropriate referrals for quality issues to promote optional outcomes.
Identifies and makesappropriate risk management referrals and provides for patient safety.
Supports, role models, and educatespatient and family on recommended infection control practices.
Documents in Case Managementsystems and the medical record.
Assesses appropriateness of admission anddetermines correct patient status using payer criteria and
clinical assessment.
Screens all patients toidentify the need for case management services.
Gathers relevant, comprehensive information and datarequired for assessment and obtainsinformation by interviewing the patient/family, the physician, other members of the healthcare team andappropriate individuals.
Prioritizesneeds ofpatient/family based onpatient’s goals.
Evaluates objectively and critically all information related to the current or proposed treatment plan to
identify potential barriers, clarify ordetermine realistic goals andobjectives, andseekappropriatealternatives for the patient.
Ensure the patient’s involvement in assessment and planning and provides and encouragesopportunities for the patient to communicate and collaborate with the case manager and the healthcareteam.
Engages the patient/family in the planning process as the primary decision maker and goal setter andcommunicates directly with the patient/familyin order to understand and incorporate priorities of thepatient d) Educates and prepares the patient/family to make informed andappropriate decisions orprovides educational resources.
Job Requirements:
Bachelor's Degree in Nursing
Ky RN license
Fiveyears direct clinical experience in an acute care hospital
Preferred Requirement:
