Position Summary:
True Care Solutions is actively seeking experienced Per Diem Infusion Registered Nurses (RNs) to provide high-quality infusion therapy in patients’ homes across thefive Boroughs, Nassau, Suffolk, Westchester, and Upstate New York.
This role is ideal for nurses who are flexible, independent, and experienced in infusion therapy. Nurses must be willing and able to travel throughout the service area based on patient needs.
You will work directly with specialty pharmacies and Certified Home Health Agencies (CHHAs) to deliver critical clinical care to patients transitioning from hospital to home.
Compensation: $55-70/hour
Reporting to: Clinical Nurse Supervisor
Key Responsibilities:
- Administering IV medications, including chemotherapy, IVIG, and monoclonal antibodies.
- Perform start of care and follow-up visits.
- Serve as liaison between pharmacy, CHHA, and care team to coordinate all aspects of patient care.
- Provide skilled infusion therapy in the home setting.
- Managed vascular access devices, including peripheral IVs and central venous catheters (CVCs) such as PICC lines, MediPorts, Hickman, and Broviac catheters, providing insertion (peripheral IVs), maintenance, medication administration, dressing changes, flushing, and monitoring for complications.
- Educate patients and families about infusion therapy, and CVC management.
- Conduct ongoing patient assessments, including dressing changes, phlebotomy, and care management.
- Communicate changes in patient condition promptly and effectively to physicians and interdisciplinary care teams.
- Maintain accurate, timely, and thorough clinical documentation in compliance with agency policies and regulatory standards.
- Ensure strict adherence to HIPAA regulations and uphold patient privacy and confidentiality standards.
Cases May Include:
- Pediatrics and Adult patients
Qualifications:
- Minimum of two (2) years of infusion nursing experience in an acute care hospital setting or comparable clinical environment.
- Current IV Therapy Certification.
- Bilingual proficiency is strongly preferred (Spanish, Russian, Mandarin, etc.)
- Current Basic Life Support (BLS). Advanced Cardiovascular Life Support (ACLS) is strongly preferred.
- Demonstrated knowledge of federal HIPAA regulations and commitment to maintaining patient privacy and confidentiality.
- Ability to accurately interpret and implement physician orders, treatment plans, and other clinical documentation.
- Strong ability to collaborate effectively as part of an interdisciplinary healthcare team.
- Excellent communication, organizational, and critical-thinking skills with the ability to provide safe, patient-centered care.
Job Requirements:
- Current and unrestricted New York State Registered Nursing (RN) License and Registration Certificate.
- Current Basic Life Support (BLS) certification required; Advanced Cardiovascular Life Support (ACLS) certification preferred.
- Current IV Therapy Certification preferred; however, certification may be substituted with documented proof of hospital-based or home infusion IV therapy experience demonstrating competency in intravenous therapy skills.
- Up-to-date physical examination and tuberculosis (TB) clearance, including QuantiFERON test, two-step PPD, or chest X-ray documentation, as applicable.
- Proof of immunity through MMR and Varicella titers/laboratory reports or documented vaccination records.
- Hepatitis B vaccination series completion, positive titers, or signed declination form.
- Urine Drug Screen (UDS) completed within the past twelve (12) months.
- Current influenza vaccination or signed declination documentation.
- Two (2) forms of government-issued identification.
- Two (2) satisfactory professional references.
- Reliable transportation required for assigned patient visits and clinical responsibilities.
- Active professional malpractice insurance coverage required.
