Pride Health is actively hiring a Physical Therapist to join our client's Home Health Team.
 
Location: Annapolis, MD 21401
Work Address: Annapolis, MD 21401
Pay Rate: Up to $61.00/hour (W-2)
Contract Length: 90 Days (ASAP Start)
Patient Care Setting: Home Health
Hours: 40 Hours Per Week
Schedule: Monday-Friday, 8:00 AM - 4:30 PM
Productivity Expectation: 30 Points Per Week
On-Call Requirement: None
Weekend Requirement: None
Coverage Areas: Prince George's County and Anne Arundel County (territories assigned separately, not on the same day)
Position Overview:
  • We are seeking an experienced and compassionate Physical Therapist (PT) to provide skilled rehabilitation services to patients in their homes throughout Prince George's and Anne Arundel Counties.
  • The Physical Therapist will evaluate, develop, and implement individualized treatment plans to improve mobility, restore function, increase independence, and enhance patients' quality of life.
Key Responsibilities:
  • Perform comprehensive physical therapy evaluations and assessments in the home setting.
  • Develop, implement, and update individualized treatment plans based on physician orders and patient needs.
  • Evaluate and treat medical-surgical, orthopedic, neurological, and mobility-impaired patients in their homes.
  • Conduct recertification visits, reassessments, and discharge evaluations as appropriate.
  • Monitor patient progress and modify treatment plans to achieve optimal outcomes.
  • Educate patients and caregivers on therapeutic exercises, mobility techniques, fall prevention, safety awareness, and home exercise programs.
  • Collaborate with physicians, nurses, occupational therapists, speech therapists, and other interdisciplinary healthcare team members.
  • Complete accurate and timely clinical documentation in accordance with agency, state, and federal requirements.
  • Meet established productivity standards while maintaining high-quality patient care.
Required Qualifications:
  • Active Maryland Physical Therapist (PT) License.
  • Graduate of an accredited Physical Therapy program.
  • Home Health experience required.
  • Ability to independently evaluate and treat patients in a home-based setting.
  • Current driver's license, reliable transportation, and automobile insurance.
  • Strong clinical assessment and patient education skills.
Preferred Qualifications:
  • Homecare Homebase (HCHB) experience.
  • OASIS assessment experience.
  • Previous Home Health care management experience.
  • Strong interpersonal and communication skills.
  • Excellent organizational and time-management abilities.
  • Ability to build strong relationships with patients, caregivers, and healthcare professionals.
