Role Overview

As a Patient Navigator, you will play a vital role in coordinating access to health-related services by assisting clients in utilizing available resources. You will support individuals in need by providing guidance, education, and social support, ensuring they receive the care and services necessary to improve their health outcomes. Your work will directly impact the well-being of our community, making a meaningful difference in the lives of those we serve.

What You Will Do

  • Identify potential clients, complete initial needs assessments, and maintain follow-up to ensure appropriate guidance to health services.
  • Provide informal counseling, education, and social support to clients.
  • Assist clients with utilizing available resources, including scheduling medical appointments and accompanying them to appointments.
  • Help clients complete medical program applications.
  • Develop and maintain strong working relationships with external and internal contacts.
  • Maintain accurate client records by documenting all contacts, services provided, and outcomes.

What You Will Need

Required Qualifications

  • Ability to identify potential clients and complete needs assessments.
  • Experience in assisting clients with resource utilization and medical program applications.
  • Proficiency in maintaining accurate client records and documentation.

Preferred Qualifications

  • Experience working with diverse populations and community agencies.
  • Familiarity with healthcare navigation and resource coordination.
