The Social Service Worker provides emergency and non-emergency direct assistance to eligible clients (resident and non-resident) for food, clothing, and household items, as well as financial assistance with rent and utilities, etc.; researches and provides client referrals to county resources; interviews clients to determine need and eligibility to receive services; prepares accurate and complete client records; refers clients to community agencies and maintains effective working relationships with the same; ensures compliance with program policies and procedures.

Greets and interviews clients requesting assistance to determine their eligibility for service based on program guidelines and restrictions; records clients’ disposition and other pertinent information such as income, expenses, family and work history; assists clients in completing applications for assistance; verifies social security cards and photocopies picture identification cards, payroll statements, utility bills, etc.Â

Assists eligible clients needing transportation assistance due to medical or relocation needs; arranges transportation through the bus line and contacts family members and churches for help.Â

Serves as liaison to other agencies to obtain/coordinate assistance on client's behalf; builds and maintains professional working relationships with community agencies.Â

Initiates intake and case plans for Center of Hope clients; instructs clients in completing the necessary paperwork, including goals and budget forms; completes orientation check-off form and release of information forms when appropriate; completes social history on new clients each week.Â

Initiates and prepares case records and logs on all Center of Hope assigned clients; ensures the accuracy and completeness of the same; enters pertinent information into the Homeless Management Information System (HMIS).

Meets regularly with clients to discuss and evaluate their progress before transferring clients to Center of Hope Case Management Specialist; prepares accurate and up-to-date records documenting the same.

Refers clients to appropriate Corps programs or other agencies based on the client’s needs and the program plan. Connects clients with Corps Officers in the local area.

Makes client referrals and maintains awareness of clients’ progress as directed by the Lead Case Manager.Â

Performs data entry of client records; prepares and files client records promptly, ensuring all information is accurate and up to date; obtains clients’ signatures as needed; reviews and tracks all client records to determine if the client is active or inactive; maintains confidentiality of client information.Â

Prepares and maintains statistical records on all services provided; compiles and prepares monthly statistical reports; ensures their accuracy and completeness.Â

Takes phone calls courteously and tactfully; assists clients by providing accurate and complete information regarding the Social Service Program operations and services.Â

MINIMUM QUALIFICATIONS REQUIRED:
Education and Experience
- Bachelor’s in social work or related field.
- Any equivalent combination of training and experience that provides the required knowledge, skills, and abilities.Â
