Full time schedule / On-site Position

Ridgway Post Acute is seeking acaring and organized Social Services Assistant to support our residents and families. This is a great opportunity for someone who enjoys helping others and wants to grow in the healthcare field while making a real difference in residents’ lives.

You’ll work closely with ourSocial Services Director and interdisciplinary team to help residents access resources, coordinate services, and maintain the highest level of well-being.

What You’ll Do

• Assist the Social Services Director withsocial service programs and resident support
• Help coordinateappointments, services, and community resources
• Supportresident and family communication
• Assist withdischarge planning and care coordination
• Maintainsocial service documentation and reports

What We’re Looking For

• Associate’s or Bachelor’s Degree preferred (not required)
• Healthcare experience preferred
• Strongcommunication and organizational skills
• Passion forhelping seniors and supporting families
