Sentara Heart Hospital located on the campus of Sentara Norfolk General Hospital in Norfolk, Virginia is hiring a Transplant Coordinator Registered Nurse for the Advanced Heart Program!
The Transplant Coordinator plays a critical role in facilitating the complex logistics of organ transplantation while prioritizing patient safety and well-being while ensuring a smooth and efficient journey for both the donor and recipient.
The Transplant Coordinator coordinates the clinical management of the transplant/VAD patient including the transplant/VAD evaluation, the transplant/VAD process and follow-up care. Provides clinical leadership to care team members, allowing staff to plan, direct, manage outcomes and evaluate care. Â
Advanced Heart Failure Unit:
- 24 bed Specialty Unit that provides high acuity cardiac care to patients with severe heart failure, pre/post heart transplant, the Total Artificial Heart, and pulmonary hypertension.
- Work with high tech equipment such as mechanical circulatory support including ventricular assist devices (LVAD, BiVAD) and telemetry monitors.
- Environment relies on 24 hours consistent communication and intricate interdisciplinary teamwork skills.
- Nurses handle 3-4 patients on this unit based on acuity level.
Education
- Bachelor of Science Nursing- BSNÂ (required)
Certification/Licensure
- Virginia RN License Virginia or Compact/Multi-State Eligible.
- All Direct Care RN's required to have BLS within 90 days of hire.
- Certified Clinical Transplant Coordinator (CCTC) required within three (3) years of hire.
Experience
- Â Two (2) years of related nursing or transplant experience required.
