Sentara is hiring a Pharmacist, Supervisor for the Outpatient Sentara to Home Pharmacy team in Hampton Roads, VA.
Location:Home office at Sentara Leigh, Norfolk, VA
Status:Full Time (40 hrs/wk)

**Must be willing to travel to all seven hospitals to round and oversee operations throughout Hampton Roads:

The Pharmacy Supervisor provides formal leadership to pharmacy team members. Responsible and accountable for the provision of safe, compliant, effective, and prompt pharmacy services. Participates in all necessary aspects of the medication-use system while providing comprehensive and individualized pharmaceutical care to the patients. Assumes leadership accountability for daily pharmacy operations, coordination of workflows, quality and service, human resource management, strategic planning, and medication management locally and within the system. Serves as a resource for staff, represents pharmacy leadership within the division, and implements and supports local and system initiatives. Assumes local and system leadership responsibility and accountability for all aspects of pharmacy operations.
Sentara To Home pharmacies are designed to help patients transition to home by assisting with medication needs before leaving the hospital. The program is designed to make the transition to home as simple and convenient as possible for patients and their families.
At Sentara To Home, medications, home medical equipment, and other healthcare needs can be provided to be prepared after discharge.
Demonstrates the ability to work in areas of pharmacy practice competently and independently
Manages patient drug therapy in accordance with applicable clinical practice standards
Assures the appropriateness of all provider medication orders in maintaining a high level of patient care and safety
Maintains advanced knowledge of medication use, disease states, and regulations applicable to their area of practice
Serves as the primary resource for drug information and education
Provides in-depth patient counseling when required
Supports the training of new employees and participates in the creation and maintenance of training materials
Consistently follows and assist with development of policies and procedures
Continually seeks out additional training opportunities and stays abreast of new and changing information for assigned areas
Demonstrates superior customer service, effective communication, decision-making, and organization
Serves as a role model in the following areas: professionalism, collaboration, and teamwork
Triages and escalates any questions or issues outside the scope of practice as needed
Uses appropriate systems for processing of patient information
Ensures quality assurance is maintained according to departmental and legal requirements
Precepts pharmacy students and/or pharmacy residents as assigned
Provides leader and peer coaching and constructive feedback to colleagues in a professional and productive manner
Successfully leads local and system implementation of various system initiatives
Shares information routinely with local and system pharmacy team members
Leads and coordinates the development and maintenance of clinical and operational pharmacy processes and programs
Develops, implements and evaluates pharmacy services, protocols, policies & procedures
Participates in local and/or system committees related to pharmacy services as necessary and assigned
Ensures adequate supplies and staffing are available for continuous patient care
Operates pharmacy equipment appropriately
Provides operational support and staffing based on the needs of the team
Models practice consistent with organizational goals, customer service, clinical effectiveness, and standards of care
In addition to the above job responsibilities, other duties may be assigned
Education
Bachelors Level Degree in Pharmacy (Required)
Doctorate Level Degree in Pharmacy (Preferred)
Certification/Licensure
Virginia Pharmacist License (Required)
Experience
Minimum Three years of Pharmacy Experience (Required)
Ambulatory/Outpatient Pharmacy Experience (Highly Preferred)
Previous Leadership Experience (Preferred)
