Sentara is hiring a Care Coordinator for our Population Health Ambulatory Clinical Support Team
Location:Sentara Corporate Facilities- Virginia Beach, VA
Status:Full Time (40 hrs/wk)
Shift:Days
Hours:8am-5pm
*This Hybrid position requires 2-3 days per week in the office at Sentara Corporate Facilities, 1330 Sentara Park, in addition to days on site at affiliate practices, and allows for some work from home days.
The Care Coordinatorperforms a variety of casework duties and provides case management services to patients, families, and designated caregivers. Must develop, participate and monitor multidisciplinary collaboration of services to patients where appropriate. Assist adult patients and their families with personal and environmental difficulties associated with medical condition up to and including at time of terminal illnesses.
Education
- Bachelor's Level Degree (Required)
Certification/Licensure
** Ideally seeking a licensed Pharmacy Technician with a Certified Medical Assistant (CCMA) or LPN**
Virginia Pharmacy Tech License and CPhT License (Received through the NHA or PTCB)
OR
Certified Medical Assistant (CMA), or Registered Medical Assistant (RMA), or Certified Clinical Medical Assistant (CCMA)
OR
LPN Virginia state license
Experience
1-year of Long-Term Care experience (Required)
Ambulatory and Clinical Pharmacy experience (Preferred)
1-Year Medicaid or Health Plan experience (Preferred)
