Sentara Health Plans is hiring a Care Coordinator/ Licensed Practical Nurse (LPN) for LOCERI/LTSS Waiver - Remote in Virginia
Status: Full-time, permanent position (40 hours)
Standard working hours: 8am to 5pm EST, M-F
Location: This position is remote in Virginia only.
Job responsibilities:
Support the Case Management team by reviewing completed LOCERI (annual assessments) to ensure accurate scoring and that all required elements are completed.
Maintain a productivity target of 12–15 reviews per day.
Education
Associate's degree in Nursingpreferred.
Bachelor's degree in Health & Human Services-related field or related experience in lieu of education accepted.
Licensed Practical Nurse (LPN) or Bachelor's prepared health professional stronglypreferred.
Certification/Licensure
LPN license in VA or Compact/Multistatepreferred
Experience
3 years Managed Care experience REQUIRED
Case Management experiencepreferred
Long-Term Services and Supports (LTSS) waiver experiencepreferred
