San Bruno Skilled Nursing is seeking a compassionate and experiencedSocial Services Director to join our dedicated team. In this role, you will advocate for residents' social, emotional, and psychosocial well-being while collaborating with families, healthcare providers, and community resources to ensure the highest quality of care.

Key Responsibilities

• Assess residents' social, emotional, and family needs and develop individualized care plans.
• Assist residents in achieving the highest practicable level of independence and well-being.
• Provide medically related social services to support residents' physical, mental, and psychosocial health.
• Offer counseling and emotional support to residents and families coping with illness, disability, grief, and life transitions.
• Coordinate community resources and agency referrals to meet resident needs.
• Assist residents and families with healthcare decisions and discharge planning.
• Coordinate outpatient appointments and ancillary services, including dental, optometry, podiatry, and psychiatric care.
• Maintain accurate and timely social services documentation.
• Collaborate with interdisciplinary team members to ensure comprehensive resident care.
• Assist with resident belongings management and related concerns.
• Provide staff education and in-service training as needed.
• Foster positive relationships with residents, families, staff, and community partners.

Qualifications

• Bachelor's Degree in Social Work, Human Services, or a related field required.
• Minimum of 2 years of supervised social work experience in a healthcare setting.
• MSW preferred.
• Skilled nursing or long-term care experience preferred.
• Experience with PointClickCare (PCC) preferred.
• Excellent communication, organizational, problem-solving, and interpersonal skills.
• Ability to maintain confidentiality and exercise professional judgment.

Physical Requirements

• Frequent standing, walking, reaching, pushing, and pulling.
• Ability to lift up to 25 pounds.
• Occasional stooping, kneeling, crouching, balancing, and climbing.
