Make a lasting impact by joiningProvidence at Home with Compassus, leaders in home-based hospice and palliative care.Weāreseeking a compassionate and experiencedNurse Practitionerto provide essential support to patients and families across various care settings, including homes, long-term care facilities, and hospitals.The Hospice and Palliative Care Nurse Practitioner (NP) functions independently as an advanced nursing professional and has responsibility to coordinate and assess care needs, create andmodify the plan of care, makeappropriate referrals for patients in collaboration with the hospice and palliative care interdisciplinary team.The Nurse Practitioner works in conjunction with the interdisciplinary team to complete all regulatory requirements such as provider face2 face, end of life visits, and assessments for re-certification. The NP, under direct physician order or by physician approved protocols and within their scope of practice may independently order opioids,adjusting dosages and change other drugs, as patient condition requires. Other responsibilitiesinclude on call coverage rotation with the other providers. In addition to the clinical duties, the NP is a primary link and resource to patients, families, and staff for education related to hospice and palliative care. Theymaintain schedules and productivity standards, document andmaintain data on all patientsreferred andseen, and participate in quality improvement activities as it relates to hospice and palliative care.Ā

Position Specific ResponsibilitiesĀ
ā¢Complies with organization-wide specific policies and procedures, including working knowledge of policies, reference to policies as needed and consistent compliance with same.Ā
ā¢Complies with patient care requirements of all regulatory agencies including working knowledge of policies as needed and compliance with same.Ā
⢠Actively creates an environment of safe delivery of patient care and functions with a constant awareness of safety issuesidentified includingimmediately correcting/reporting any unsafe conditions found.Ā
⢠Under required physician supervision independently conducts a thorough medical, functional, and psychosocial assessment on assigned patients; performs and interprets diagnostic tests and procedures as defined by protocol; assesses whether patientās condition requires the immediate attention of the physician.Ā
⢠Providesappropriate medical intervention when needed,initiates and alters certain drug therapies, and writes orders according to established protocols approved by the physician or direct physician orders.Ā
⢠Performs Face 2 Face, End of Life, and recertification assessment visits.Ā
⢠Collaborates with other members of the health team in providing continuity of care to meet the unique needs of the patient; provides patient information to the physician.Ā
ā¢Assists the physician by ordering drugs and supplies, keeping records, as defined by protocol.Ā
ā¢Participates in documenting notes in the clinical medical record.Ā
ā¢Participates in interdisciplinary team meetings, recommendscourse oftreatment andmonitors ongoing care.Ā
⢠Provides liaison and information to physicians, discharge planning, and other departmentsregardingquality of care issues.Ā
⢠Recommendsappropriate action based on the patientās condition.Ā
⢠Under direct physician order or by physician approved protocols independently order IV therapy, including antibiotics and opioids, alteringdosages and changing to other drugs, as patient condition requires.Ā
ā¢Furnishes medications per standardized procedures including medications available under Providence St.Ā
⢠Josephs Health formulary or those medications usually prescribed by specialty physicians.Ā
⢠Ordering feedings andplacement/replacement of nasogastric or replacement of gastric tube, adjusting rate and formula based on patientās needs, asdetermined by nutritional need and/or swallowing capabilities.Ā
ā¢Assists family and patientsregarding decisions including goals and values, withdrawal and withholding of interventions, non-beneficialinterventions and resuscitation status.Ā
⢠Educates the patient and familyregarding diagnosis of medical condition and prognosis.Ā
ā¢Assists in the referral of patients toappropriate health facilities and community resources.Ā
⢠Provides education to nursing staff/nursing facility departments, and family membersregarding the special needs of the patient and elderly population.Ā
ā¢Establishes and maintains courteous cooperative relations with the public,patients and other personnel.Ā
ā¢Participates in the development and implementation of education based onevidence based practices.Ā
⢠Performs other duties asrequired.Ā
ā¢Demonstrates working knowledge of the Hospital Information System.Ā
⢠Performs documentation in patientās chart which istimely,accurate and concise per organization policies/procedures and regulations.Ā
⢠Utilizescomputer based communications anddata base system.Ā
ā¢Participate in Provider on call rotation.Ā
⢠Performs other duties as assigned.Ā

Education and/or ExperienceĀ
Required - Master's Degree NursingĀ Ā
Required - 2 years Acute or transitional care settingĀ Ā

Certifications, Licenses, and RegistrationsĀ
Required - An active and unencumbered RN license in the state of practice.Ā Ā
Required upon hire: National Provider BLS - American Heart Association (Vendor Managed)Ā

Physical Demands and Work Environment:The demands of this rolenecessitate a team member to effectively perform essential functions. Adaptations can be made to accommodate team members with disabilities. Regular standing, walking, and manual dexterity are fundamental, along with the ability to lift and move objects up to 50 pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus. In a healthcare setting, exposure to bodily fluids, infectious diseases, and conditions typical to the fieldis expected. Routine use of standard medical equipment and tools associated with clinical care is essential. Compensation range:$56.93-$73.67
The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities
