As a Community Health Worker, you will serve as a trusted link between patients, healthcare providers, and community organizations. You will help individuals navigate healthcare and social service systems, identify and address barriers to care, promote health education, coordinate services,andsupportpatientsinachievingtheirpersonalhealthgoals.Throughoutreach,advocacy,andcarecoordination,youwillempowerindividualstotakeanactiveroleintheirhealthcarejourneywhilecontributingtohealthiercommunitiesacrosstheregion.

KeyResponsibilities:
·Coordinatecareandsupportservicesforindividualsbyconnectingthemwithhealthcareproviders,behavioralhealthservices,andcommunityresources.
·Buildmeaningfulrelationshipswithpatientsandempowerthemtoachievetheirhealthandwellnessgoals.
·Educateindividualsandfamiliesonhealthcareresources,preventativecare,wellnessinitiatives,andavailablesupportprograms.
·Conductoutreachactivitiesthroughphonecalls,communityvisits,homevisits,andcommunityeventstoengageindividualsincareandservices.
·Identifybarrierstocareandcollaboratewithpatientsandinterdisciplinaryteamstodevelopeffectivesolutions.
·Advocateforpatientswhilepromotingculturallyresponsive,person-centeredcare.
·Assistindividualswithunderstandinginsurancecoverageoptionsandcompletingeligibilityandenrollmentapplications,includingQualifiedHealthPlans(QHPs).
·Collaboratewithhealthcareproviders,careteams,andcommunitypartnerstoimprovecarecoordinationandhealthoutcomes.
·Maintainaccuratedocumentation,records,anddataentryinaccordancewithorganizationalrequirements.
·Supportdepartmentalinitiatives,qualityimprovementefforts,andotherassignedprojects.
Education,Experience,and/orCredentialQualifications:
·HighSchoolDiplomaorequivalentrequired.
·Bachelor'sdegreeinHealthSciences,SocialWork,HumanServices,PublicHealth,orarelatedfieldpreferred.
·Two(2)yearsofexperienceincommunityhealth,patientnavigation,healthcareoutreach,humanservices,casemanagement,orarelatedfieldpreferred.
·Knowledgeofcommunityhealthprinciples,socialdeterminantsofhealth,harmreductionpractices,andmotivationalinterviewingtechniques.
·Knowledgeoflocalcommunityresources,supportprograms,andsocialservicesystems.
·Strongcustomerservice,relationship-building,communication,andproblem-solvingskills.
·ProficiencywithMicrosoftOfficeapplicationsincludingExcel,Outlook,Teams,andWord.
·Abilitytoworkindependentlywhilecollaboratingeffectivelywithinamultidisciplinaryteam.
AdditionalQualifications:
·Validdriver'slicense,reliabletransportation,andcurrentautoinsurancerequired.
·Excellentinterpersonal,activelistening,andcommunicationskills.
·Strongorganizationalskillswithattentiontodetail.
·Abilitytocommunicateeffectivelywithdiversepopulationsinaclear,compassionate,andprofessionalmanner.
·Experienceincustomerservice,peersupport,coaching,communityoutreachandengagement,healthcareorbehavioralhealthsettings,publicspeaking,andhealtheducationisbeneficial.
·CompleteanapprovedCommunityHealthWorkertrainingprogramthrougharecognizedcurriculumprovider.
·MustcompleterequiredtrainingrelatedtoTrauma-InformedCare,BehavioralHealthConditionsinChildren,SuicidePrevention,andSubstanceUseDisorders.
·MustattainFirstAidCertificationwithinthree(3)monthsofhireandmaintaincertificationthereafter.
·MustattainCPRCertificationwithinthree(3)monthsofhireandmaintaincertificationthereafter.
·Tuberculosis(TB)screeningrequiredforalldirectcarestaff.
Physical Requirements
· Perform sedentary work requiring exertion of up to 10 pounds of force occasionally and negligible force frequently
· Sit for extended periods while providing services, completing documentation, and attending meetings
· Occasionally walk and stand while conducting client services and participating in program activities
· Perform repetitive hand, finger, and arm movements associated with typing, writing, and electronic documentation
· Lift, carry, push, or pull light objects as necessary to perform essential job functions
· Utilize computers, office equipment, and electronic health record systems throughout the workday
