APP CORE JOBSUMMARY                                                                           

The Advanced Practice Provider (APP) delivers medical care to a wide variety of patients. The APP also examines and treats patients independently and in autonomous collaboration with other health care professionals, ensures proper illness and injury care and disease prevention, diagnosis, treatment, and recovery. May prescribe medications and order diagnostic tests. Advises patients about continuing care.

APP CORE JOBFUNCTIONS                                

• Performs and documents complete physical examinations and comprehensive healthhistories.

• Functions independently to perform age-appropriate history and physical forpatients.

• Orders and interprets diagnostic and therapeutic tests relative to patient’s age-specificneeds.

• Prescribes appropriate pharmacologic and non-pharmacologic treatmentmodalities

• Implements interventions to support the patient to regain or maintain physiologicstability.

• Assists with the provision of care in accordance with facility, state, and federalregulations.

• Monitors the effectiveness ofinterventions.

• Facilitates the patient’s transition within and between health care settings, e.g. admitting, transferring, and discharging patients.

• Collaborates with multidisciplinary team members by making appropriatereferrals.

• Facilitates staff, patient and family decision making by providing educationaltools.

• Adheres to University and unit-level policies and procedures and safeguards Universityassets.

COREQUALIFICATIONS                                                                                     

Education:

Master’s degree in relevantfield

Certification andLicensing:

Valid State of Florida APRN License or PALicense

Experience:

Minimum 2 years of relevantexperience

 DEPARTMENT OF HEMATOLOGY SPECIFIC JOBFUNCTIONS                            

The primary duties and responsibilities of the Hematology Advanced Practice Provider includes, but are not limited to:

Accountabilities:

Clinical (Both Ambulatory andInpatient)

• Assesses the physical and psychosocial status of clients by means of interview, health history, physical examination, and diagnostic studies.

• Recognizes deviations from normal in the physical assessment. Works in collaboration with a physician in formulating treatment plans for health problems and follow-up.

• Writes prescriptions for medications and blood products based upon laboratory results, orders and interprets routine diagnostic and follow-up studies, therapeutic measures, and post- discharge care in accordance with written practice protocols.

• Requests written consultation from physicians and other healthcare professionals to ensure appropriate and quality patient care.

• Assesses and follows consult patients when required and independently of physician whenneeded.

• Interprets and evaluates findings of studies/tests and intervenesaccordingly.

• Advises collaborating physician of appropriate information regarding patient care and reports to the physician the actions provided to the patients.

• Evaluates the quality of care provided and recommends changes forimprovement.

• Initiates appropriate actions to facilitate the implementation of therapeutic plans that are consistent with the continuing healthcare needs of the clients.

• Answers patient-related inquiries via all forms of telecommunication throughout the system (Telephone, Email, Secure Chat, In-basket messaging, etc.)

• Conducts or participates in multidisciplinary rounds and ensures appropriate and quality care for patients.

• Performs patient triage and determines the urgency of physician’sevaluation.

• Provides symptom management for side effects related to hematologic oncologictreatment.

• Assist with Peer-to-Peer authorizations and filling out medical forms forpatients.

• Ensure that the clinic runs smoothly and efficiently, corresponding with referring physicians and patients to provide adequate coordinate of care to improve outcomes.

• Performs bone marrow biopsies, lumbar punctures with IT chemo, Omaya reservoir access, PICC line removals and skin biopsies if competency met.

• Act as a preceptor for APP students, fellows and new APP staff asappropriate.

AmbulatorySpecific:

• Rotates with colleagues taking weekend and holiday call from Sylvesterpatients.

• Responds to medical and oncologic emergencies in the CTU andclinics.

• Communicates with triage nurses regarding patients who need to be added to OCC clinic for symptom management, evaluation, and treatment.

InpatientSpecific:

• Interacts with admitting physician, hospitalist, and other multidisciplinaryteams.

• Prioritizes consults to present to attending physician and rounds with attending daily on patients as appropriate.

• Writes consult inpatient notes, discharge notes and dictates asappropriate.

• Rotates with colleagues performing Hematology consultations and inpatient care at UMH as per divisional needs.

Research

• CITI certification within three months ofemployment

• Facilitates clinical research protocols and coordinates patient care with researchstaff.

• Review clinical trial options with patients andfamilies.

• Interview and examine researchsubjects.

• Evaluate research subjects’ appropriateness for studyinclusion/enrollment.

• Follow all regulatory requirements for human subjects’ research compliance anddocumentation.

• Monitor, Review study related labs and imaging, and provide ongoing follow up to hematology patients on clinical trials.

• Triage research patientcommunication.

• Report SAEs and AEs to designated researchstaff.

• Communicate with the PIs and Sub PIs with questions or concerns regarding holding or continuing investigational or standard of care treatment.

• Perform study related procedures if competencymet.
