The Nurse Care Manager works closely with internal and external treatment team members to provide education, coordination, and support to individuals who haveorareatriskforcomplexmedicalconditions.Thispositionplaysakeyroleintreatmentplanning,chronicdiseasemanagement,healtheducation,caretransitions,andimprovingoverallhealthoutcomesthroughperson-centerednursingcareandcollaboration.

KeyResponsibilities:

·Developandimplementwellness,prevention,andhealtheducationinitiativesforindividualswithcomplexmedicalneeds.

·Facilitatehealtheducationgroupsandprovidetrainingonmedicaldiagnoses,treatmentplans,medications,andchronichealthconditions.

·Collaboratewithclientsandtreatmentteammemberstodevelopindividualizedhealthcaregoalsandtreatmentplans.

·ParticipateintheinitialtreatmentplanningprocessforHealthcareHomeenrollees.

·Consultwithmultidisciplinarytreatmentteamsregardingidentifiedhealthconcernsandchronicdiseasemanagement.

·Coordinatehospitaladmissions,discharges,andtransitionsofcarewithmedicalprovidersandhealthcaresystems.

·Monitor,track,andreportperformancemeasuresandhealthoutcomesusingspecializednursingandhealthcareinformationsystems.

·Implementandsupportrequiredhealthtechnologyprogramsandinitiatives,includingCyberAccess,CareManager,andIntellectAbility.

·Respondappropriatelytoemotionalcrisesbyaccessingemergencyservicesandcoordinatingrapidassistancewhenneeded.

·Participateinmeetings,qualityassuranceactivities,programevaluationefforts,andotherassignedinitiativesinaccordancewithorganizationalstandards.

Education,Experience,and/orCredentialQualifications:

·ActiveRegisteredNurse(RN)licenserequired.

·ExperienceworkingwithindividualswithIntellectualandDevelopmentalDisabilities(IDD)preferred.

·Knowledgeofchronicdiseaseprocessesaffectingchildrenandadults.

·Strongunderstandingofnursingprocesses,carecoordination,andpatient-centeredcare.

AdditionalQualifications:

·Currentdriver'slicense,acceptabledrivingrecord,andcurrentautoinsurancerequired.

·Excellentwrittenandverbalcommunicationskills.

·Abilitytoimplementinnovativesystemsandprocesses.

·Abilitytomakesoundclinicaldecisionsusingcriticalthinkingandnursingjudgment.

·Abilitytoworkcollaborativelywithinternalandexternalhealthcarepartners.

·Successfulcompletionofbackgroundcheck,includingcriminalrecord,drivingrecord,abuse/neglectscreening,andfingerprintcheck.

PhysicalRequirements

·Performsedentarywork,exertingupto10poundsofforceoccasionallyandnegligibleforcefrequentlytomoveobjects.

·Utilizerepetitivehand,finger,andarmmovementsfortyping,documentation,andwrittencommunication.

·Remainseatedforextendedperiodsthroughouttheworkday.
