Position Summary

We are seeking a skilled and compassionateOccupational Therapist (OTR) to providepatient-centered therapy services on a PRN basis. In this role, you’ll help residents improvefunction, independence, and quality of life while collaborating with an interdisciplinary care team.

Key Responsibilities

• Evaluate residents and develop individualizedtreatment plans
• Provide therapy services perphysician orders and care goals
• Document evaluations, treatments, and progress notes accurately and timely
• Collaborate with physicians, nursing, and therapy team members
• Educate residents, families, and caregivers on therapy programs
• Participate incare planning and discharge planning
• Conductin-services and staff training as needed
• Ensure compliance with allstate, federal, and payer guidelines

Qualifications

• Degree in Occupational Therapy (Bachelor’s required; Master’s/Doctorate preferred)
• ActiveSouth Carolina OT license (OTR) in good standing
• Experience inskilled nursing or rehab setting preferred
• Familiarity withPointClickCare (PCC) and Casamba preferred
• Strong documentation, communication, and clinical skills
